Obesity and respiratory pathology

ABSTRACT

Introduction: the interrelations of obesity with respiratory pathology are complex, currently explained in terms of new pathogenic theories. In children, obesity has a negative impact on quality of life, both by the appearance of comorbidities, and the difficulties of therapy associated pathologies.

Purpose: to evaluate the clinical and epidemiological point of view, respiratory comorbidities in a group of obese children.

Material and methods: the study group consisted of 110 children diagnosed with overweight. They evaluated the presence of asthma, gastroesophageal reflux disease, atopic dermatitis and allergic mucosal manifestations.

Results: the incidence of respiratory events in obesity is high, meeting the third of the children. In 31% of cases there is a real association between obesity and asthma, gastroesophageal reflux was found in fourth and fifth events in patients allergic to. Obesity changes the negative development of chronic respiratory pathology in children of school age, intervening negative quality of life.

OBEZITATEA SI PATOLOGIA RESPIRATORIE

REZUMAT

Introducere: interrelaţiile obezităţii cu patologia respiratorie sunt complexe, explicate în prezent din perspectiva noilor teorii patogenice. La copil, obezitatea are impact negativ asupra calităţii vieţii, atât prin apariţia comorbidităţilor, cât şi prin dificultăţile de terapie a patologiilor asociate.

Scopul studiului: evaluarea din punct de vedere clinico-epidemiologic a comorbidităţilor respiratorii la un lot de copii obezi.

Material şi metode: lotul de studiu a fost format din 110 copii diagnosticaţi cu exces ponderal. S-au evaluat prezenţa astmului bronşic, a bolii de reflux gastroesofagian, a dermatitei atopice şi a manifestărilor mucoase alergice.

Rezultate: Incidenţa manifestărilor respiratorii în obezitate este mare, întâlnindu-se la o treime din copii. În 31% dintre cazuri există o asociere reală între obezitate şi astm, refluxul gastroesofagian fiind întâlnit la un sfert, iar manifestările alergice la o cincime din pacienţi. Obezitatea modifică în sens negativ evoluţia patologiei respiratorii cronice la copilul de vârstă şcolară, intervenind nefavorabil asupra calităţii vieţii.

Therapy with biological agents in juvenile-onset ankylosing spondylarthritis

REZUMAT

Ankylosing spondylarthritis is an immune-mediated inflammatory disease, which includes axial and peripheral arthritis, inflammation of the site where tendons and joint capsules attaches to the bone and degeneration of the sacroiliac joint and the spinal cord joints, leading to progressive spinal fusions. Introduction of new immunomodulatory molecules into therapy aiming at interleukins, such as the tumor necrosis factor alpha, was considered a major breakthrough in the treatment of severe ankylosing spondylarthritis. This case illustrates the particular evolution of the clinical manifestations in correlation with joint trauma, gastrointestinal infection and the devastating consequences of active HLA-B27 positive ankylosing spondylarthritis with juvenile onset.

REZUMAT

Spondilartrita anchilozantă este o boalǎ inflamatorie mediată imun, care include artrita axială şi periferică, inflamaţia locului unde tendoanele şi capsulele articulare se inseră pe os, precum şi degenerarea articulaţiilor sacro-iliace şi ale coloanei vertebrale, care conduce la fuziuni progresive ale vertebrelor. Introducerea în terapie a moleculelor imunomodulatoare, cu ţintă pe interleukine, cum ar fi pe factorul de necroză tumorală alfa, a fost considerată ca un progres major în tratamentul formelor severe de spondiliartrite ankilozante. Acest caz ilustrează evoluţia particulară a manifestărilor clinice în corelaţie cu traumatismul articular, infecţia digestivă şi consecinţele devastatoare ale spondilitei ankilozante active HLA-B27 pozitivă cu debut juvenil.

Ursodeoxycholic acid benefi t in pediatric non-alcoholic fatty liver disease

REZUMAT

Introduction. Nonalcoholic fatty liver disease (NAFLD), the most common cause of chronic liver disease in children, encompasses a range of disease from simple steatosis to steatohepatitis (NASH). Ursodeoxycholic acid (UDCA) has been used in adult pacients with NASH, demonstrating its efficacy in high doses.

Purpose. The aim of our study was to assess the effect of high-dose ursodeoxycholic acid therapy on liver functions in children with NASH.

Methods. We prospectively studied 18 children with NASH aged 2.5 to 17 years. NASH was operationally defined as diffusely hyperechogenic liver at ultrasonography with persistently elevated (> 35 UI/l) alanine aminotransferase (ALT) or aspartate aminotransferaze (AST) after exclusion of viral, alcohol-induced, druginduced, cholestatic and genetic causes of liver disease. Patients were treated with ursodeoxycolic acid (20 mg/ kg per day) for six month.

Results. Treatment with UDCA significantly decreased hepatic transaminases. ALT levels ranged from 68.5 (50-166) at baseline vs. 47 (29-104) after six month, p <0.001, AST from 43 (29-101) 34 (17-75), p < 0.001, and GGT between 41.5 (11-150) vs. 22 (8-55). At the end of treatment, ALT levels normalized in 4 patients. Moreover, cholesterol levels were significantly reduced after treatment.

Conclusions. Treatment with high-dose ursodeoxycholic acid improves liver function parameters in children with NASH.

BENEFICIUL ACIDULUI URSODEOXICOLIC IN STEATOZA HEPATICA NON-ALCOOLICA LA COPIL

REZUMAT

Introducere. Boala ficatului gras non-alcoolic (NAFLD), cea mai frecventă cauză a bolii hepatice cronice la copii, cuprinde un spectru de afecţiuni variind de la steatoză simplă până la steatohepatită (NASH). Acidul ursodeoxicolic (UDCA) a fost utilizat la pacienţi adulţi cu NASH, demonstrându-şi eficacitatea în doze crescute.

Scop. De a evalua efectul tratamentului cu doze crescute de UDCA asupra parametrilor funcţiei hepatice la copii cu NASH.

Metode. Lotul de studiu a fost constituit din 18 copii cu NASH cu vârste cuprinse între 2,5 şi 17 ani. NASH a fost operaţional definit ca ficatul difuz hiperechogenic la ecografie cu valori persistent crescute (> 35 UI/l) ale alanin aminotransferazei (ALAT) sau aspartat aminotransferazei (AST), după excluderea cauzelor virale, drog sau alcool induse, colestatice sau genetice ale bolii hepatice. Pacienţii au fost trataţi cu acid ursodeoxicolic (20 mg/kg/zi), timp de şase luni.

Rezultate. Tratamentul cu UDCA a determinat scăderea semnificativă a valorilor transaminazelor hepatice. Nivelurile ALT au variat între 68,5 (50-166) iniţial vs 47 (29-104) după şase luni, p < 0,001, AST între 43 (29-101) vs 34 (17-75), p < 0,001, şi GGT între 41,5 (11-150) vs. 22 (8-55). La sfârşitul tratamentului, niveluri ALT s-au normalizat la 4 dintre pacienţi. De asemenea, concentraţiile de colesterol au fost semnificativ reduse după tratament.

Concluzii. Tratamentul cu doze crescute de acid ursodeoxicolic imbunătăţeşte parametrii funcţiei hepatice la copii cu NASH.

Adrenogenital syndrome in newborn

ABSTRACT

Severe acute dehydration in infants may occur in the context of multiple etiological causes the most common were acute gastroenteritis. But resistance to the treatment of electrolyte and acid base correction by the persistence of hyponatremia and associations with other clinical signs suggestive, undertake further investigations for the adrenogenital syndrome or “loss of salt”. Diagnosis and treatment of this constitutes an emergency because of risk of death in these infants are exposed. The authors present the case of a newborn derived from the twin pregnancy diagnosed with adrenogenital syndrome and its evolutive and treatment particularities.

SINDROM ADRENOGENITAL LA NOU-NASCUT

REZUMAT

Deshidratarea acută severă la sugar poate să survină în contextul unor multiple cauze etiologice, cea mai frecventă fiind gastroenterita acută. Rezistenţa la tratamentul de corecţie electrolitic şi acidobazic prin persistenţa hiponatremiei alături de alte semne clinice sugestive obligă însă la investigaţii suplimentare pentru sindromul adrenogenital sau „de pierdere de sare“. Diagnosticul şi tratmentul acestuia constituie o urgenţă prin riscul de deces la care sunt expuşi aceşti sugari. Autorii prezintă cazul unui nou-născut provenit din sarcină gemelară diagnosticat cu sindrom adrenogenital şi particularităţile evolutive şi de tratament ale acestuia.

Clinical and evolutive aspects in IgE-mediated food allergy in infants and small children

ABSTRACT

The skin, respiratory and gastrointestinal allergy manifestations can evolves alternatively or concomitant in the first years of life; food sensitization is the common etiology. The objective of this study is the assessment of IgE-mediated food allergy regarding clinical manifestations and their correlation with risk factors, the dietary and drug therapy impact during a 22 months monitoring. Results. The onset food allergy manifestations (mean age 5-11 months) involve skin (66 cases), respiratory (55 cases) and/or gastrointestinal tract (5 cases). We evidenced respiratory aeroallergens sensitization (bronchial asthma and/or allergic rhinitis) in 50 percents of cases. 43 children achieved oral tolerance for one or more foodS, most commonly in milk and egg white allergy. Conclusions. The secondary and tertiary food allergy prevention measures are not allways be able to prevent the progression of atopic march to polisensitizations and complex clinical manifestations.

ASPECTE CLINICO-EVOLUTIVE SUB TRATAMENT IN ALERGIA ALIMENTARA IgE-MEDIATA LA SUGAR SI COPILUL MIC

REZUMAT

Manifestările alergice cutanate, respiratorii şi/sau digestive din primii ani de viaţă pot evolua eşalonat, concomitent sau alternativ, sensibilizarea alimentară fiind cauza cea mai frecventă.

Obiectivul studiului a fost evaluarea alergiei alimentare IgE mediate la sugar şi copilul mic privind formele şi gravitatea manifestărilor clinice în corelaţie cu factorii de risc şi impactul prognostic al măsurilor dietetice şi medicamentoase evaluat în dinamică, individualizat după o dispensarizare medie de 22 de luni.

Rezultate. Debutul real al alergiei alimentare a fost precoce (5-11 luni) prin manifestări cutanate (66 cazuri), respiratorii (55 cazuri) şi/sau digestive (5 cazuri). Apariţia manifestărilor respiratorii (astm bronşic şi/sau rinită alergică) prin sensibilizare la aeroalergeni a fost surprinsă în 50% din cazuri. Toleranţa orală pentru unul sau mai multe alimente s-a instalat la 43 copii, mai frecvent pentru lapte şi albuş de ou.

Concluzie. Măsurile de profilaxie secundară şi terţiară aplicate susţinut la copiii cu manifestări alergice corelate cu sensibilizarea alimentară nu împiedică evoluţia spre polisensibilizări şi manifestări clinice complexe în toate cazurile
