DIAGNOSTICUL POZITIV AL ALERGIEI ALIMENTARE MEDIATE IgE LA COPIL

REZUMAT

Diagnosticul alergiei alimentare este dificil în pofida progreselor recente înregistrate în descifrarea patogeniei bolii. Suspiciunea clinică necesită confirmare printr-o baterie de investigaţii (anamneză, examen fizic, jurnal alimentar, teste cutanate alergologice, determinarea IgE serice totale şi specifice, investigarea leziunii ţesutului intestinal prin biopsie şi/sau testul permeabilităţii intestinale, alte teste folosite în special în scop de cercetare: eliberarea de histamină din bazofile şi mastocitele intestinale, determinarea complexelor imune circulante, testul transformării limfoblastice, dozarea triptazei şi a proteinei cationice a eozinofilului). Proba de provocare alimentară dublu orb controlată placebo reprezintă gold standardul diagnostic în alergia alimentară la copil, dar nu se poate efectua decât în centre specializate de către personal instruit în acest sens, fiind inaccesibilă practicii curente.

Scleroderma in young children

ABSTRACT

Chronic disease of unknown etiology, scleroderma represents a disease that occurs rarely in young children, with an estimated frequency of less than 1% up to age 5. Evolution of scleroderma is variable, unpredictable and the delineation of clinical forms in children is difficult. Some patients may experience periods of calm for many years, others have a progressive worsening, without remission and death. Our case is characterized by the onset at a young age, bringing together major and minor criteria for systemic scleroderma.

SCLERODERMIA LA COPILUL MIC

REZUMAT

Boală cronică de etiologie necunoscută, sclerodermia este o boală care apare rar la copilul mic, estimându-se o frecvenţă de sub 1% până la vârsta de 5 ani. Evoluţia cazurilor de sclerodermie este variabilă, imprevizibilă, delimitarea formelor clinice fiind dificilă la copil. Unii bolnavi pot prezenta perioade de acalmie mai mulţi ani, alţii au o evoluţie progresivă spre agravare (fără remisiuni) şi deces. Cazul prezentat se particularizează prin debutul sclerodermiei la vârstă mică, bolnavul întrunind, la 1 an si 3 luni, criterii majore şi minore de sclerodermie sistemică.

Pathogenic pathways of drug therapy in child’s asthma

ABSTRACT

Asthmatic airways inflammation with still unknown various pathways involves multiple components and is orchestrated by numerous cell types, particularly mast cell, eosinophils, and CD4+ lymphocytes, but also airway structural cells. By there activation release over 100 pro-inflammatory mediators and cytokines, which in turn cause vascular leakage, bronchial smooth muscle contraction, inflammatory cell infiltration, mucus hypersecretion, airway hyper-responsiveness (AHR), and not ultimately airway remodeling. These changes are expressed clinically as recurrent wheezing, dyspnea, chest tightness and cough, which are typically associated with reversible airflow obstruction. Inhaled corticosteroids (ICS) are recommended as first-line treatment of persistent disease, but not all patients achieve asthma control even with high doses ICS or in combination with other medications, including a long acting β2 agonist or a leukotriene modifier. Such patients may require additional therapy. Last years, as we progress in knowledge asthma physiopathology, several novel anti-inflammatory therapies are in different stages of clinical development. The most clinically advanced of these is Omalizumabul (IgE antibody). Other key therapies options in clinical development either target pro-inflammatory cytokines or inflammatory cells.

BAZELE PATOGENICE ALE TRATAMENTULUI MEDICAMENTOS IN ASTMUL BRONSIC LA COPIL

REZUMAT

Inflamaţia cronică caracteristică astmului bronşic implică multiple componenete şi este orchestrată de numeroase tipuri celulare, în mod particular de mastocite, eozinofile şi limfocitele CD4+, dar şi de celulele structurale ale căilor respiratorii. Activarea acestor celule conduce la eliberarea a peste 100 de mediatori proinflamatori şi citokine, care determină edem, bronhoconstricţie, infiltrare celulară, hipersecreţie de mucus, hiper-reactivitate bronşică (HRB) şi, nu în ultimul rând, remodelarea bronşică. Aceste modificări se exprimă clinic prin wheezing, dispnee şi tuse şi se asociază în mod caracteristic cu obstrucţia reversibilă a căilor respiratorii. Controlul inflamaţiei alergice în astm se obţine folosind corticosteroizi inhalatori (CSI) ca primă linie terapeutică, dar nu toţi pacienţii răspund, chiar şi atunci când sunt folosiţi în doze mari sau în combinaţie cu alte medicamente, inclusiv β2 agonişti cu durată lungă de acţiune şi/sau antileucotriene. Astfel de pacienţi au nevoie de terapie adiţională. În ultimii ani, pe măsură ce s-a înaintat în cunoaşterea patogeniei astmului, s-au dezvoltat câteva noi terapii antiinflamatorii, cea mai avansată fiind terapia cu Omalizumab (anticorpi antiIgE). Alte opţiuni terapeutice au ca target citokinele pro-inflamatorii sau celulele inflamatorii.

Quality of life assessment – an important part of the food allergy management in children

ABSTRACT

The prevalence of food allergy is increased during the last decades (25-30%); it is involved in skin and respiratory allergic diseases. Elimination diet of the food identified through history and specific tests represents the only benefic therapeutic measure. The quality of life’s impact of food allergy is influenced by the necessity of a continuous vigilance for careful reading of the labels of the commercial foods, recognition of the less known terms, correction of the nutritional deficiencies due to the absence of an essential food, increased price of the hypoallergenic formula, other allergic comorbidities and parents’ concerns of fatal allergic reaction.

EVALUAREA CALITATII VIETII – COMPONENTA IMPORTANTA A MANAGEMENTULUI ALERGIEI ALIMENTARE LA COPIL

REZUMAT

Alergia alimentară se înscrie pe panta ascendentă a prevalenţei alergiilor (25-30%), fiind implicată etiopatogenetic în boli cutanate şi/sau respiratorii. Dieta de eliminare a alimentului identificat prin anamneză şi investigaţii specifice ca fiind responsabil pentru simptomatologie reprezintă singura măsură terapeutică cu rezultate benefice. Calitatea vieţii copiilor şi familiei este afectată de necesitatea unei vigilenţe crescute pentru citirea atentă a etichetelor produselor comerciale şi recunoaşterea termenilor mai puţin cunoscuţi, corectarea deficienţelor nutriţionale datorate absenţei alimentelor esenţiale din dietă, costul ridicat al formulelor hipoalergenice, existenţa altor comorbidităţi alergice, îngrijorarea unei posibile reacţii alergice fatale.

Juvenile idiopathic arthritis – risk factors and clinical forms

ABSTRACT

The aim of this article is to determine the JIA’s risk factors and the prevalence of clinical forms, poiting out the difficulties of situating them at onset as well as in evolution taking into consideration a significant number of patients (169 cases). The outlined clinical forms (in order of frequency) were: oligoarthritis (44,17%), RFnegative polyarthritis (33.13%), enthesitis (11.24%), systemic arthritis (5.91%), RF-positive polyarthritis (2, 95%) and psoriatic arthritis (2.36%), with the percentage values close to those of other studies, except the RF-negative polyarthritis subtype in which the frequency is higher. Individualized analysis of cases revealed real time from onset of disease to diagnosis and risk factors for JIA (in order of frequency): traumatic (9,46%), positive family history (5,32%) and infections (2,35%). Also, it emphasized the possibility of transforming a clinical form to another even under adequate treatment.

REZUMAT

Obiectivul este de a stabili factorii de risc şi prevalenţa formelor clinice de AJI cu evidenţierea dificultăţilor de încadrare la debut şi în evoluţie luând în studiu un lot semnificativ de bolnavi (169 cazuri). Formele clinice conturate (în ordinea frecvenţei) au fost: oligoarticulară (44,17%), poliarticulară cu FR negativ (33,13), entezită (11,24%), sistemică (5,91%), poliarticulară cu FR pozitiv (2,95%) şi psoriazică (2,36%). Analiza individualizată a cazuisticii a scos în evidenţă durata de la debutul real al bolii până la diagnostic, cât şi factorii de risc pentru AJI (în ordinea frecvenţei): traumatici (9,46%), antecedente familiale pozitive (5,32%) şi infecţioşi (2,36%). De asemenea, a fost evidenţiată posibilitatea transformării unei forme clinice în alta chiar sub tratament adecvat.

Risk factors for asthma in children (6-18 years)

ABSTRACT

Asthma is one of the most common diseases in children and an important cause of morbidity and mortality, with recent increased prevalence. Asthma diagnosis in children is often underestimated in early childhood. The aim of present study is to anamnestically assess using clinical and specific paraclinic exploration the incidence of the risk factors highly suggestive of asthma in children (6-18 years) and the correlation with disease severity. The questionnaires used in this study included relevant information to known risk factors for asthma during pre and postnatal period, preadolescence and adolescence stage. The principal risk factors for asthma identified in descending order of frequency were: personal atopy, recurrent wheezing, male gender, urban environment, indoor air allergens exposure (house dust, domestic animals, house mold) allergic sensitization to indoor allergens (house-dust mites), infant artificial feeding, family atopy, exposure to second hand tobacco smoke. Allergic polisensitization and tobacco smoke exposure are associated with severe asthma. The majority of cases were affected by asthma-related comorbid conditions and allergic rhinitis was the most frequent comorbidity.
