DEFICITUL DE FIER SI ANEMIA PRIN DEFICIT DE FIER – PROBLEMA GLOBALA DE SANATATE PUBLICA

REZUMAT

Deficitul de fier este definit ca o stare în care nu există un nivel corespunzător de fier necesar pentru a menţine funcţiile normale fiziologice ale organismului. Anemia este o problemă globală de sănătate publică, de proporţiile unei epidemii, care afectează atât ţările în curs de dezvoltare cât şi ţările dezvoltate, cu consecinţe majore pentru sănătatea umană, precum şi dezvoltarea socială şi economică. În prezent, deficitul de fier şi în special anemia prin carenţă de fier rămâne una dintre deficienţele nutriţionale cele mai grave şi importante din lume. Fiecare grupă de vârstă este vulnerabilă. Sugarii, copiii mici, preşcolarii şi adolescenţii prezintă un risc mai mare de a dobândi deficit de fier, de aceea Organizaţia Mondială a Sănătăţii (OMS) a elaborat un pachet cuprinzător de măsuri de sănătate publică care abordează toate aspectele legate de un deficit de fier şi anemia feriprivă. În încercarea de a diminua prevalenţa deficitului de fier şi a anemiei feriprive, Societatea Europeană de Gastroenterologie, Hepatologie şi Nutriţie Pediatrică (The European Society for Paediatric Gastroenterology Hepatology and Nutrition-ESPGHAN) a sintetizat în 2014 recomandările utilizate şi în prezent privind profilaxia marţială.

IRON DEFICIENCY AND IRON DEFICIENCY ANEMIA – A GLOBAL PUBLIC HEALTH PROBLEM

ABSTRACT

Iron deficiency is defined as a condition in which there is not an adequate iron level necessary to maintain the normal physiologic functions. Having the magnitude of epidemics, anemia is a global public health problem, which affects both the emerging countries and the developed ones, with major consequences for human health and for social and economic development. Nowadays, iron deficiency and especially iron-deficiency anemia, remains one of the most severe and important nutritional deficiencies. Each group of age is vulnerable. Infants, small children, pre-school children and teenagers show a higher risk of iron deficiency. Accordingly, the World Health Organization developed an ample package of public health measures that approaches all the aspects related to iron deficiency and iron-deficiency anemia. Trying to diminish the prevalence of iron deficiency and iron deficiency anemia, the European Society for Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) has synthesized in 2014 the recommendations used also at present for martial prophylaxis.

DERMATITA ATOPICA – DISFUNCTIA IMUNA ASOCIATA ATOPIEI

REZUMAT

Sistemul imunitar joacă un rol complex în apărarea organismului ca răspuns la „non-self“, răspunde anormal la antigenii alergeni (hipersensibilitate şi autoimunitate) şi este caracterizat prin toleranţă imună cu lipsa de reactivitate la propriile structuri (auto).

Scop. Scopul acestui studiu este de a demonstra că, în dermatita atopică, deficitul imun influenţează dezvoltarea atopiei, severitatea bolii şi dezvoltarea de comorbidităţi.

Material şi metode. În urma analizei foilor de observaţie, 135 de cazuri diagnosticate cu AD au fost incluse în studiu. Analiza statistică a fost realizată cu ajutorul SPSS V20 pentru determinarea frecvenţei şi testarea ipotezelor, pentru p <0,05, prin testele T şi OneWay ANOVA.

Rezultate. Din cele 135 de cazuri, 51,9% au fost copii de sex masculin şi 48,1% de copii de sex feminin, cu vârsta cuprinsă între 1 lună şi 127 luni, cu o medie de 26,21. Potrivit IgE serice totale, 64,4% dintre pacienţi au avut niveluri ridicate de IgE, 35,6% niveluri normale. În conformitate cu SCORAD, copiii au avut forma uşoară în 20,7% din cazuri, forma moderată în 70,4%, iar severă în 8,9% din cazuri. Deficitul de IgA a fost găsit în 48,1% din cazuri, iar deficitul de IgG a fost găsit în 38,5% din cazuri. Testele t de corelaţie pe eşantioane independente constată valori semnificative statistic între nivelul IgE şi deficitul de IgA, între SCORAD şi deficitul de IgG şi IgA. Marşul atopic este influenţat de valorile crescute ale IgE şi deficitul de IgA pentru un p<0,05.

Concluzii. Atopia în DA poate fi influenţată de factori complecşi, atât interni, cât şi de mediu, dar acest lucru rămâne un subiect controversat. Factorii externi care acţionează pe un fundal genetic predispus la atopie poate declanşa manifestarea DA.

ATOPIC DERMATITIS – ASSOCIATED IMMUNE DYSFUNCTION

ABSTRACT

The immune system shows a complex role to defend the body in response to “non-self” antigens, respond abnormally to antigens allergens (hypersensitivity and autoimmunity) and shows immune tolerance by lack of reactivity to its own structures (self).

Aim. The aim of this study is to demonstrate that in atopic dermatitis immune deficiency influences the development of atopy, disease severity and comorbidities.

Material and methods. Following medical record review, 135 cases diagnosed with AD were included in the study. Statistical analysis was performed using SPSS v20 for determining the frequency and testing the hypotheses, for p < 0.05, by t tests and One-Way ANOVA.

Results. Of the 135 cases, 51.9% were male children and 48.1% female children aged 1 month to 127 months with a mean of 26.21. According to total serum IgE level, 64.4% of patients had elevated IgE levels, 35.6% normal levels. According to the SCORAD, children had mild AD in 20.7% of cases, moderate in 70.4%, and severe in 8.9%. IgA deficiency was found for 48.1% of cases, and for 51.9% normal. IgG deficiency was found in 38.5% of cases. The independent samples t tests showed statistical significant demonstrating correlations between IgE level and IgA immune deficiency, between SCORAD and IgG and IgA immune deficiency. Atopic march is influenced by elevated IgE, IgA and IgG immune deficiency, p <0.05.

Conclusions. Atopy in AD can be influenced by complex factors, both internal and environmental, but this remains a controversial topic. External factors acting on a background genetically predisposed to atopy trigger the manifestation of AD.

Obesity and respiratory pathology

ABSTRACT

Introduction: the interrelations of obesity with respiratory pathology are complex, currently explained in terms of new pathogenic theories. In children, obesity has a negative impact on quality of life, both by the appearance of comorbidities, and the difficulties of therapy associated pathologies.

Purpose: to evaluate the clinical and epidemiological point of view, respiratory comorbidities in a group of obese children.

Material and methods: the study group consisted of 110 children diagnosed with overweight. They evaluated the presence of asthma, gastroesophageal reflux disease, atopic dermatitis and allergic mucosal manifestations.

Results: the incidence of respiratory events in obesity is high, meeting the third of the children. In 31% of cases there is a real association between obesity and asthma, gastroesophageal reflux was found in fourth and fifth events in patients allergic to. Obesity changes the negative development of chronic respiratory pathology in children of school age, intervening negative quality of life.

OBEZITATEA SI PATOLOGIA RESPIRATORIE

REZUMAT

Introducere: interrelaţiile obezităţii cu patologia respiratorie sunt complexe, explicate în prezent din perspectiva noilor teorii patogenice. La copil, obezitatea are impact negativ asupra calităţii vieţii, atât prin apariţia comorbidităţilor, cât şi prin dificultăţile de terapie a patologiilor asociate.

Scopul studiului: evaluarea din punct de vedere clinico-epidemiologic a comorbidităţilor respiratorii la un lot de copii obezi.

Material şi metode: lotul de studiu a fost format din 110 copii diagnosticaţi cu exces ponderal. S-au evaluat prezenţa astmului bronşic, a bolii de reflux gastroesofagian, a dermatitei atopice şi a manifestărilor mucoase alergice.

Rezultate: Incidenţa manifestărilor respiratorii în obezitate este mare, întâlnindu-se la o treime din copii. În 31% dintre cazuri există o asociere reală între obezitate şi astm, refluxul gastroesofagian fiind întâlnit la un sfert, iar manifestările alergice la o cincime din pacienţi. Obezitatea modifică în sens negativ evoluţia patologiei respiratorii cronice la copilul de vârstă şcolară, intervenind nefavorabil asupra calităţii vieţii.
