DEFICITUL DE FIER SI ANEMIA PRIN DEFICIT DE FIER – PROBLEMA GLOBALA DE SANATATE PUBLICA

REZUMAT

Deficitul de fier este definit ca o stare în care nu există un nivel corespunzător de fier necesar pentru a menţine funcţiile normale fiziologice ale organismului. Anemia este o problemă globală de sănătate publică, de proporţiile unei epidemii, care afectează atât ţările în curs de dezvoltare cât şi ţările dezvoltate, cu consecinţe majore pentru sănătatea umană, precum şi dezvoltarea socială şi economică. În prezent, deficitul de fier şi în special anemia prin carenţă de fier rămâne una dintre deficienţele nutriţionale cele mai grave şi importante din lume. Fiecare grupă de vârstă este vulnerabilă. Sugarii, copiii mici, preşcolarii şi adolescenţii prezintă un risc mai mare de a dobândi deficit de fier, de aceea Organizaţia Mondială a Sănătăţii (OMS) a elaborat un pachet cuprinzător de măsuri de sănătate publică care abordează toate aspectele legate de un deficit de fier şi anemia feriprivă. În încercarea de a diminua prevalenţa deficitului de fier şi a anemiei feriprive, Societatea Europeană de Gastroenterologie, Hepatologie şi Nutriţie Pediatrică (The European Society for Paediatric Gastroenterology Hepatology and Nutrition-ESPGHAN) a sintetizat în 2014 recomandările utilizate şi în prezent privind profilaxia marţială.

IRON DEFICIENCY AND IRON DEFICIENCY ANEMIA – A GLOBAL PUBLIC HEALTH PROBLEM

ABSTRACT

Iron deficiency is defined as a condition in which there is not an adequate iron level necessary to maintain the normal physiologic functions. Having the magnitude of epidemics, anemia is a global public health problem, which affects both the emerging countries and the developed ones, with major consequences for human health and for social and economic development. Nowadays, iron deficiency and especially iron-deficiency anemia, remains one of the most severe and important nutritional deficiencies. Each group of age is vulnerable. Infants, small children, pre-school children and teenagers show a higher risk of iron deficiency. Accordingly, the World Health Organization developed an ample package of public health measures that approaches all the aspects related to iron deficiency and iron-deficiency anemia. Trying to diminish the prevalence of iron deficiency and iron deficiency anemia, the European Society for Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) has synthesized in 2014 the recommendations used also at present for martial prophylaxis.

ALIMENTATIA DIVERSIFICATA – SCURTA TRECERE IN REVISTA A UNOR PRINCIPII CORECTE PENTRU O SANATATE PE TERMEN LUNG

REZUMAT

Nutriţia reprezintă totalitatea proceselor fiziologice de asimilare şi degradare a alimentelor care au loc într-un organism, necesare creşterii şi dezvoltării. Se menţin de actualitate discuţiile asupra tendinţelor în alimentaţie, numeroase societăţi ştiinţifice şi comitete internaţionale dedicându-şi atenţia asupra impactului pe care alimentaţia corectă o are asupra creşterii şi dezvoltării armonioase, începând încă din viaţa intrauterină. Introducerea progresivă în alimentaţia sugarului, după vârsta de 4-6 luni, a alimentelor semisolide şi solide, ce înlocuiesc treptat prânzurile de lapte administrate anterior este cunoscută sub denumirea de alimentaţie diversificată (complementară). Ultimele recomandări ale Comitetului ESPGHAN (European Society for Pediatric Gastroenterology, Hepatology and Nutrition) privind momentul trecerii de la o alimentaţie exclusiv lactată la una care să conţină şi alte alimente sunt pentru intervalul de vârstă cuprins între săptămânile 17-26. O importanţă primordială o are complianţa şi colaborarea familiei cu medicii. Având în vedere posibilitatea actuală de a accesa numeroase informaţii, uneori fără o bază ştiinţifică, am considerat oportună o scurtă trecere în revistă a principalelor aspecte ale unei alimentaţii diversificate conform recomandărilor de specialitate.

COMPLEMENTARY FEEDING – A SHORT REVIEW OF SEVERAL CORRECT PRINCIPLES OF LONG-TERM HEALTH

REZUMAT

Nutrition epitomizes the whole range of physiological processes of absorbtion and decomposition of food in the body, as required for growth and development. The debates on the feeding trends are topical, a large number of scientific boards and international committees fully concentrate on the impact of correct feeding on growth and harmonious development, starting even from intrauterine life. The progressive introduction of half solid and solid food in the infant’s diet after 4-6 months, gradually replacing the milk meals previously administered is known as diversified (complementary) feeding. The latest recommendations of the ESPGHAN Committee (European Society for Pediatric Gastroenterology, Hepatology and Nutrition) regarding the moment of switching from an exclusively milk diet to a diet which includes other foods is included in the period of time between week 17 and 26. The compliance and the collaboration of the family with the doctors are of paramount importance. Considering the present day open access to a lot of information, sometimes without any scientific basis, we believe it is opportune to review briefly the main aspects of the complementary feeding according to the recommendations in the field.

IMPACTUL INFECTIEI CU STAPHYLOCOCCUS AUREUS ASUPRA EVOLUTIEI FIBROZEI CHISTICE LA COPIII DINTR-UN CENTRU REGIONAL DIN NORD-ESTUL ROMANIEI

REZUMAT

Introducere. Infecţiile bacteriene cronice şi, adesea, exacerbările infecţioase acute sunt caracteristice bolii pulmonare din fibroza chistică (FC). Obiectivul studiului a fost reprezentat de identificarea copiilor cu FC, depistarea infecţiei cu Staphylococcus aureus la aceşti bolnavi şi cuantificarea impactului infecţiei asupra statusului clinic.

Material şi metodă. S-a realizat un studiu prospectiv pe o perioadă de 3 ani pe un lot de 37 de copii cu FC aflaţi în evidenţa Centrului Regional din Clinica III Pediatrie Iaşi, România. La aceşti bolnavi s-a urmărit prezenţa S. aureus, prevalenţa tulpinilor de S.aureus meticilino-rezistente (SAMR), impactul infecţiei cronice cu SAMR asupra funcţiei pulmonare concomitent cu prezenţa unor elemente de severitate în FC.

Rezultate. 22 de pacienţi dintre cei 37 cu FC au prezentat culturi pozitive pentru S. aureus; la 9 dintre aceştia s-a izolat SAMR, 5 având infecţie cronică. Grupul SAMR pozitiv a prezentat valori semnificativ mai mici ale volumului de gaz expirat în prima secundă a expirului forţat (FEV1), status nutriţional mai precar, bronşiectazii severe, un număr mai mare de exacerbări pulmonare şi implicit de spitalizări, insuficienţă pancreatică în majoritatea cazurilor şi o asociere mai mare cu genotipul F508del.

Concluzii. S. aureus rămâne un patogen important în FC. Din cauza patogenicităţii sale crescute, S. aureus şi în special tulpinile SAMR pot contribui la evoluţie clinică nefavorabilă.

THE IMPACT OF THE STAPHYLOCOCCUS AUREUS INFECTION ON THE EVOLUTION OF CHILDREN WITH CYSTIC FIBROSIS FROM A REGIONAL CENTRE IN NORTH-EASTERN ROMANIA

ABSTRACT

Introduction. Chronic bacterial infections and – often – acute infectious exacerbations are characteristic to the lung disease of the cystic fibrosis (CF). The objective of this study was to identify children with CF and Staphylococcus aureus infection and quantify the impact of this infection on their clinical status.

Material and methods. A prospective study was carried out during 3 years on a batch of 37 children with CF registered in the records of the Regional Monitoring Centre of the 3rd Clinic of Pediatrics of Iasi, Romania. In these patients, we searched for the presence of S. aureus, the prevalence of methicillin-resistant S. aureus (MRSA) strains, the impact of the chronic infection with MRSA on the lung function concomitant with the presence of severity elements in CF.

Results. 22 of the 37 patients with CF had positive culture for S. Aureus; at 9 of them was isolated MRSA and 5 had chronic infection. The MRSA positive group of patients presented signifi cantly lower values of the FEV1, a more poor nutritional status, severe bronchiectases, a larger number of pulmonary exacerbations and implicitly hospitalizations, pancreatic insufficiency in most cases and a higher degree of association with the F508del genotype.

Conclusions. S. aureus remains an important pathogen in CF. Due to its high pathogeny, S. aureus and especially the MRSA strains can contribute to an unfavourable clinical evolution.

REZUMAT

Greutatea mică la naştere este un indicator important al sănătăţii infantile prin prisma relaţiei strânse dintre greutatea la naştere şi morbiditatea şi mortalitatea infantilă. Diminuarea ratei de mortalitate şi morbiditate presupune cunoaşterea particularităţilor de creştere şi îngijire la aceste categorii de copii. Indiferent de categoria din care fac parte – prematuri, mici pentru vârsta gestaţională, întârziere în creşterea intrauterină – aceşti copii, prin greutatea la naştere mică, au un risc crescut de a dezvolta malnutriţie, motiv pentru care la aceştia se impune cunoaşterea necesarului energetic şi de principii alimentare maximale, particular lor. Asigurarea unui suport nutriţional optim precoce reprezintă elementul esenţial al îngrijirii acestora.

CURRENT TRENDS IN FEEDING AND CARING FOR PREMATURE AND LOW BIRTH WEIGHT INFANTS

ABSTRACT

Low weight at birth is an important indicator of infant health due to the tight relationship between birth weight and infant mortality and morbidity. The diminishing of the mortality and morbidity rate requires information on the growth characteristics and caring requirements for this category of children. Regardless of the category they are in – prematures, Small for Gestational Age, delay in intrauterine growth – these children, due to their low weight at birth present a high risk to develop malnutrition, a reason why there should be known the energy requirements and the optimum nutrition principles, specific to them. Ensuring an early most favourable diet is the essential element in their care assistance.

CURRENT TRENDS IN FEEDING AND CARING FOR PREMATURE AND LOW BIRTH WEIGHT INFANTS

ABSTRACT

Low weight at birth is an important indicator of infant health due to the tight relationship between birth weight and infant mortality and morbidity. The diminishing of the mortality and morbidity rate requires information on the growth characteristics and caring requirements for this category of children. Regardless of the category they are in – prematures, Small for Gestational Age, delay in intrauterine growth – these children, due to their low weight at birth present a high risk to develop malnutrition, a reason why there should be known the energy requirements and the optimum nutrition principles, specific to them. Ensuring an early most favourable diet is the essential element in their care assistance.
