DEFICITUL DE FIER SI ANEMIA PRIN DEFICIT DE FIER – PROBLEMA GLOBALA DE SANATATE PUBLICA

REZUMAT

Deficitul de fier este definit ca o stare în care nu există un nivel corespunzător de fier necesar pentru a menţine funcţiile normale fiziologice ale organismului. Anemia este o problemă globală de sănătate publică, de proporţiile unei epidemii, care afectează atât ţările în curs de dezvoltare cât şi ţările dezvoltate, cu consecinţe majore pentru sănătatea umană, precum şi dezvoltarea socială şi economică. În prezent, deficitul de fier şi în special anemia prin carenţă de fier rămâne una dintre deficienţele nutriţionale cele mai grave şi importante din lume. Fiecare grupă de vârstă este vulnerabilă. Sugarii, copiii mici, preşcolarii şi adolescenţii prezintă un risc mai mare de a dobândi deficit de fier, de aceea Organizaţia Mondială a Sănătăţii (OMS) a elaborat un pachet cuprinzător de măsuri de sănătate publică care abordează toate aspectele legate de un deficit de fier şi anemia feriprivă. În încercarea de a diminua prevalenţa deficitului de fier şi a anemiei feriprive, Societatea Europeană de Gastroenterologie, Hepatologie şi Nutriţie Pediatrică (The European Society for Paediatric Gastroenterology Hepatology and Nutrition-ESPGHAN) a sintetizat în 2014 recomandările utilizate şi în prezent privind profilaxia marţială.

IRON DEFICIENCY AND IRON DEFICIENCY ANEMIA – A GLOBAL PUBLIC HEALTH PROBLEM

ABSTRACT

Iron deficiency is defined as a condition in which there is not an adequate iron level necessary to maintain the normal physiologic functions. Having the magnitude of epidemics, anemia is a global public health problem, which affects both the emerging countries and the developed ones, with major consequences for human health and for social and economic development. Nowadays, iron deficiency and especially iron-deficiency anemia, remains one of the most severe and important nutritional deficiencies. Each group of age is vulnerable. Infants, small children, pre-school children and teenagers show a higher risk of iron deficiency. Accordingly, the World Health Organization developed an ample package of public health measures that approaches all the aspects related to iron deficiency and iron-deficiency anemia. Trying to diminish the prevalence of iron deficiency and iron deficiency anemia, the European Society for Pediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) has synthesized in 2014 the recommendations used also at present for martial prophylaxis.

REZUMAT

Boala Gaucher este cea mai frecventă maladie lizozomală, cu transmitere autosomal recesivă. Boala se datorează deficitului enzimei glucocerebrozidază, rezultând în acumularea de glucocerebrozide în toate organele. Diagnosticul se stabileşte prin măsurarea activităţii enzimei. Dintre formele clinice, tipul 2 este cel mai rar şi are prognosticul cel mai sumbru. Prezentăm cazul unui sugar depistat la vârsta de 5 luni cu simptome neurologice şi evoluţia lui ulterioară cu hepatosplenomegalie masivă, fiind diagnosticat cu tipul 2 de boală Gaucher.

TYPE 2 GAUCHER DISEASE: ONSET AND EVOLUTION – CASE REPORT

ABSTRACT

Gaucher disease is the most common lysosomal storage disorder, with autosomal recessive transmission. The disease is due to glucocerebrosidase enzyme deficiency, resulting in accumulation of glucocerebroside in all organs. The diagnosis is established by measuring enzyme activity. Among the clinical forms, type 2 is the rarest and has the most dismal prognosis. We present the case of an infant found at 5 months of age with neurological symptoms and his subsequent evolution with massive hepatosplenomegaly, being diagnosed with type 2 Gaucher disease.

INGESTIA CONCOMITENTA DE ETILENGLICOL SI ETANOL: O CAPCANA DE DIAGNOSTIC?

REZUMAT

Etilenglicolul este unul dintre cei mai toxici alcooli, fiind consumat accidental sau voluntar, ca substituent al etanolului sau în scop suicid. Ingestia de etilenglicol determină, prin metaboliţii săi toxici, acidoză metabolică severă, cu creşterea hiatusului anionic şi osmotic, conducând la un tablou clinic ce asociază deprimarea sistemului nervos central, afectare cardiovasculară şi insuficienţă renală. Prezentăm cazul unui pacient în vârstă de 16 ani internat pentru semne clinice şi biologice de intoxicaţie cu etilenglicol apărute după ingestia simultană de antigel şi alcool. Pacientul a prezentat acidoză metabolică doar la debut, rapid corectată de o doză de bicarbonat de sodiu; investigaţiile ulterioare nu au relevat acidoză, chiar dacă în evoluţie a apărut insuficienţa renală acută ce a necesitat hemodializă. Datorită absenţei anamnezei pozitive şi a acidozei metabolice persistente, diagnosticul de intoxicaţie cu etilenglicol a fost întârziat până la confirmarea prin testul toxicologic seric. Concluzii: ingestia concomitentă de etanol poate masca simptomele intoxicaţiei cu etilenglicol; absenţa acidozei metabolice persistente nu exclude diagnosticul.

CONCOMITANT INGESTION OF ETHYLENE GLYCOL AND ETHANOL: A DIAGNOSIS TRAP?

ABSTRACT

Ethylene glycol is one of the most toxic alcohols; it may be accidentally or intentionally consumed as a substitute for ethanol or related to suicidal attempts. Ingestion of ethylene glycol causes a severe metabolic acidosis with increased anion and osmotic gap due to its toxic metabolites, leading to a clinical picture of central nervous system depression, cardiovascular and renal impairment. A 16-year-old boy was admitted with clinical and biological signs of ethylene glycol poisoning after simultaneous ingestion of antifreeze and ethanol. The patient had mild anion gap metabolic acidosis only at the debut, rapidly corrected with one dose of sodium bicarbonate; further evaluation did not reveal acidosis, even if the subsequent evolution included acute renal failure requiring hemodialysis. Due to the absence of a positive history and of a persistent metabolic acidosis, the diagnosis of ethylene glycol poisoning was delayed until it was confirmed by serum toxicological test. Conclusions: concomitant ingestion of ethanol may mask the symptoms of ethylene glycol poisoning; the absence of persistent metabolic acidosis does not rule out the diagnosis.

ACCIDENT VASCULAR CEREBRAL ISCHEMIC LA UN COPIL CU CARDIOPATIE CONGENITALA CIANOGENA COMPLEXA

REZUMAT

Accidentul vascular cerebral (AVC) este rar la copil comparativ cu adultul. Cauzele cele mai comune de AVC la copil sunt cardiopatiile emboligene pentru infarctul cerebral şi malformaţiile arteriovenoase pentru hemoragiile cerebrale, dar un număr de cazuri rămân idiopatice, fără a li se putea afi rma etiologia precisă. Autorii prezintă cazul unui copil diagnosticat în perioada de sugar cu malformaţie congenitală de cord cianogenă complexă, dar neoperată, internat cu semne clinice şi radiologice de pneumonie, care a evoluat favorabil sub tratament antibiotic şi simptomatic. În timpul perioadei rezolutive a pneumoniei, copilul devine brusc somnolent, prezentând hemiplegie dreaptă cu absenţa refl exelor de aceeaşi parte. Examenul clinic corelat cu CT cranio-cerebral au stabilit diagnosticul de AVC ischemic sylvian bilateral. Evoluţia a fost favorabilă sub tratament conservator (antiagregante plachetare, fi zioterapie), persistând însă hemiplegia dreaptă.

ISCHEMIC STROKE IN A CHILD WITH COMPLEX CYANOTIC CONGENITAL HEART DISEASE

ABSTRACT

Stroke is rare in children compared to adults. Most common causes of stroke in children are heart diseases that cause embolism and cerebral infarction and arteriovenous malformations for the brain hemorrhages, but a number of cases remain idiopathic, without being able to say precise etiology. The authors present the case of a child diagnosed in infancy with complex cyanotic heart malformation, but not operated, admitted with clinical and radiological signs of pneumonia, which evolved favorably with antibiotic and symptomatic therapy. During resolution of the pneumonia, the child becomes suddenly drowsy, presenting right hemiplegia, with absence of refl exes on the same side. Clinical examination correlated with cranio-cerebral CT have established the diagnosis of bilateral Sylvian ischemic stroke. The evolution was favorable under conservative treatment (antiplatelet drugs, physiotherapy), but right hemiplegia persisted.

BIOPROTEZA VALVULARA MITRALA DEGENERATA PRECOCE

REZUMAT

Prolapsul de valvă mitrală (PVM) este destul de frecvent întâlnit la copil, fi ind deseori o descoperire întâmplătoare la un copil asimptomatic, dar şi la copii simptomatici, cu boli ale ţesutului conjunctiv. Autorii raportează cazul unei fetiţe de 11 ani, diagnosticată la vârsta de 6 ani cu insufi cienţă mitrală severă prin prolaps de valvă mitrală şi operată la vârsta de 7 ani, când s-a practicat înlocuirea valvei mitrale cu o proteză valvulară biologică. La 4 ani de la protezarea valvulară, fetiţa se prezintă în urgenţă pentru instalarea bruscă a dispneei cu ortopnee, hemoptiziei şi cianozei generalizate. Examenul clinic corelat cu ecocardiografi a au stabilit diagnosticul de edem pulmonar acut şi proteză biologică valvulară degenerată, cu stenoză mitrală severă. Evoluţia a fost favorabilă după înlocuirea în urgenţă a bioprotezei cu o proteză mecanică.

EARLY DEGENERATED BIOPROSTHETIC MITRAL VALVE

Irina-Mihaela Ciomaga, Georgiana Russu, Cristina Jitareanu, Violeta Streanga, Aniela Rugina and Nicolai Nistor

ABSTRACT

Mitral valve prolapse (MVP) is fairly common in children, often being randomly found in asymptomatic, as well as in symptomatic patients with conjunctive tissue diseases (CTD). The authors report the case of an 11-yearold girl, diagnosed at the age of 6 with MVP and severe mitral regurgitation (MR). She had surgery at the age of seven, when the mitral valve replacement with a biological valve prosthesis was performed. Four years later, the girl was admitted for sudden onset of dyspnoea with orthopnoea, haemoptysis and generalized cyanosis. The diagnosis of acute pulmonary oedema and degenerated biological valvular prosthesis correlated with severe mitral stenosis was evoked by means of clinical and echocardiographic examinations. The evolution was favourable after the replacement of the bioprosthesis with a mechanical prosthesis.
