Chronical constipation multifactorial etiopathogeny in childhood

ABSTRACT

Constipation is an apparently common condition in children, but it has important consequences in pediatric age. Its prevalence varies between 0.7 and 29.6% (with an average of 8%), with no important difference between sexes. The etiology still remains unclear, and although many mechanisms involved in this disease were discovered, it is still very heterogenous. Constipation may be functional, related with an inadequate diet or with psychical circumstances which influences the normal act of defecation, organic, in which gastrointestinal diseases and not only, play a big role, or it may be talked about a genetic determinism. Specific associated gene mutations haven’t been related yet. There are plenty of genetic syndromes in which constipation is a frequent symptom. Trying to objectify the diagnosis, many types of classifications were suggested, based on the main types of the symptoms. Despite of this, diagnosing constipation in children still remains an important challenge.

REZUMAT

Constipaţia este o patologie aparent banală, dar cu implicaţii importante la vârsta pediatrică. Prevalenţa acesteia variază de la 0,7% la 29,6% (în medie 8%), fără o diferenţă semnificativă între sexe. Etiologia constipaţiei rămâne încă neclară, fiind extrem de heterogenă în ciuda identificării a numeroase mecanisme ce stau la baza acestei afecţiuni. Constipaţia poate fi de natură funcţională, corelată cu o dietă neadecvată sau cu circumstante de natură psihică ce influenţează desfăşurarea normală a defecaţiei, organică, având la bază diferite boli ale tractului gastrointestinal şi nu numai, sau poate avea determinism genetic. Cu toate acestea, încă nu s-au găsit mutaţii genice asociate în mod specific. Numeroase sindroame genetice asociază ca mainifestare clinică constipaţia. În încercarea concretizării diagnosticului au fost propuse diferite clasificări bazate pe simptomatologie. Cu toate acestea, diagnosticul constipaţiei la populaţia pediatrică poate fi dificil de stabilit.

Diagnosis and management in chronic constipation in children age (Part II)

ABSTRACT

Constipation is a common health problem that affects children of all ages. The etiology is often functional, being the result of voluntary retention of the stool. A complete case history and a detailed physical examination are essential for an appropriate diagnosis. In most cases laboratory investigations are not required; they are indicated when a suspicion of an organic cause occurs (Hirschsprung disease, pseudoobstruction, hypothyroidism, diabetes insipidus, cystic fibrosis, congenital anorectal malformations) or in treatment-resistant cases. Early diagnosis is extremely important being associated with a better response to therapy. This includes performing DRE, colonoscopy, scintigraphy, colon videocapsule and anorectal manometry. The first stage of functional constipation treatment consists in desimpactation. Polyethylene glycol is effective and well tolerated. After desimpactation, a long-term maintenance program is necessary, due to increased frequency of relapses. Maintenance drugs include mineral oil, lactulose, polyethylene glycol and sorbitol. A diet rich in dietary fiber associated with family support and, when possible, child education are essential. The delay in diagnosis and treatment can lead to the progression into a chronic health problem with physical, psychological and social implications.

DIAGNOSTIC SI MANAGEMENT IN CONSTIPATIA CRONICA LA COPIL (Partea a II-a)

REZUMAT

Constipaţia este o problemă comună care afectează copiii de toate vârstele. De cele mai multe ori etiologia este de natură funcţională, fiind rezultatul retenţiei voluntare a scaunului. Un rol important în stabilirea diagnosticului îl au obţinerea unei anamneze complete şi efectuarea unui examen fizic detaliat. În majoritatea cazurilor investigaţiile paraclinice nu sunt necesare pentru diagnostic, ele fiind indicate atunci când există suspiciunea unei cauze organice de constipaţie (boala Hirschsprung, pseudoobstrucţie, hipotiroidism, diabet insipid, fibroză chistică, malformaţii anorectale congenitale) sau în cazurile cu rezistenţă la tratament. Diagnosticul precoce este extrem de important, fiind asociat cu un răspuns mai bun la terapie. Acesta include efectuarea tuşeului rectal, colonoscopia, scintigrafia colonică, videocapsulă şi manometria anorectală. Prima etapă a tratamentului constipaţiei funcţionale constă în dezimpactare. Polietilen glicolul este eficace şi bine tolerat. După dezimpactare este iniţiat un program de întreţinere pe termen lung, din cauza frecvenţei crescute a recidivelor, cu medicamente ca uleiul mineral, lactuloza, polietilenglicolul şi sorbitolul. Regimul alimentar bogat în fibre alimentare asociat cu educaţia familiei şi, atunci când este posibil, a copilului este esenţial. Întârzierea diagnosticului şi implicit a tratamentului poate duce la progresia spre o problemă cronică semnificativă de sănătate cu implicaţii fizice, psihologice şi sociale.

Endoscopic management of gastrointestinal bleeding in children

ABSTRACT

Gastrointestinal bleeding,a major and frequent urgency in pediatric gastroenterology can develop self-limited (75- 85% of cases) or severe, with hemodynamic compromised, when require clinical assessment, resuscitation and digestive endoscopy for diagnosis and possible treatment. Application of endoscopic parameters (Forrest classification) to assess recurrence and prognosis of gastrointestinal bleeding peg subsequent attitude. Therapeutic endoscopy within circumstances established by The North American Society of Pediatric Gastroenterology, Hepatology and Nutrition choosing one of the techniques of injection, or mechanical coagulation in an individualized manner to etiopathogeny of gastrointestinal bleeding, it decreases the number of surgical intervention and frequency of deaths. Both early endoscopic diagnosis and endoscopic haemostatic therapy with endovenous administration of proton pump inhibitors, are measures that reduce bleeding and hemorrhage recurrence rate of 80-90%. Endoscopist experience will influence the readiness of surgical recommendation in serious, life-threatening cases.

MANAGEMENTUL ENDOSCOPIC AL HEMORAGIILOR DIGESTIVE LA COPIL

REZUMAT

Hemoragia digestivă, o urgenţă majoră şi frecventă în gastroenterologia pediatrică, poate evolua autolimitat (75-85% cazuri) sau sever cu compromiterea hemodinamicii, când se impune o evaluare clinică, reanimare şi endoscopie în scop diagnostic şi eventual terapeutic. Aplicarea parametrilor endoscopici (clasificarea Forrest) pentru evaluarea recurenţei şi prognosticului unei hemoragii digestive jalonează atitudinea ulterioară. Endoscopia terapeutică în limitele circumstanţelor stabilite de declaraţia Societăţii Nord Americane de Gastroenterologie, Hepatologie şi Nutriţie Pediatrică, alegând una dintre tehnicile de injectare, coagulare sau mecanice, într-o manieră individualizată etiopatogeniei hemoragiei digestive, scade numărul intervenţiilor chi rurgicale şi frecvenţa deceselor. Diagnosticul endoscopic precoce împreună cu terapia hemostatică endoscopică şi administrarea endovenoasă de înhibitori ai pompei protonice, sunt măsuri care reduc sângerarea şi recurenţa hemoragiei în proporţie de 80-90%. Experienţa endoscopistului va influenţa promptitudinea recomandării intervenţiei chirurgicale în formele grave, cu risc vital.

REZUMAT

Consimţământul informat a devenit o componentă majoră, dar şi obligatorie în practica medicală contemporană, numeroase foruri şi publicaţii subliniind comandamentele sale etice şi psihologice, ca şi modalităţile de obţinere şi mai ales implicaţiile de ordin legal. Implicaţiile acestei proceduri în cazul pediatriei sunt complexe, câteodată confuze sau insuficient intelese. Astfel dacă în cazul consultului clinic sau al actelor medicale simple, con sim- ţământul poate fi considerat implicit, în situaţia unor manevre provocatoare de disconfort psihic sau fizic, investigaţii invazive – în care este inclusă şi endoscopia sau acte terapeutice presupunând posibilitatea unor ris curi sau complicaţii, obţinerea unui consimtământ informat din partea parinţilor sau aparţinătorilor legali şi respectiv a unui acord (accept) din partea unui copil evoluat din punct de vedere intelectual, psihologic şi social devine mandatorie. Trebuie totuşi clarificate nuanţele nu numai semantice între consimţământ şi acord, ponderea fiecaruia din acestea în decizia finală, discernământul pacientului minor la diferite grupe de vârstă şi în egală măsură rolul şi locul medicului curant în astfel de circumstanţă.

INFORMED CONSENT IN PEDIATRIC GASTROINTESTINAL ENDOSCOPY

ABSTRACT

Informed consent has become a major, but also mandatory component in medical praxis nowadays. A great number of forums and publications emphasize its ethical and psychological commands, the most adequate way to obtain it and especially its legal implications. In pediatrics, the implications of this procedure are complex, sometimes confusing or not well understood. Thus, in clinical consultations or simple medical acts, the consent could be seen as implicit; when we talk about manoeuvres that lead to psychological or physical discomfort, invasive investigations, including endoscopy or therapeutical acts with possible risks or complications, informed consent from parents or legal tutors, respectively an intellectually, psychologically and socially evolved, educated child’s consent (acceptance) becomes mandatory. We must, however, clarify the nuances between consent and acceptance, not only from the semantic point of view, the proportion of either of the two terms in the final decision, the minor patient’s ability to discern at different age stages and finally the role and the importance of the doctor in such decision makings.

ETIOPATOGENIA MULTIFACTORIALA A PURPUREI HENOCH-SCHONLEIN LA VARSTA PEDIATRICA

ABSTRACT

Purpura Henöch-Schönlein (PHS) este o afecţiune a vaselor mici caracterizată printr-un răspuns anormal al sistemului imun: imunoglobulina A (IgA), fracţiunea a 3-a a complementului şi depunerea de complexe imune circulante în pereţii arteriolelor, venulelor şi a capilarelor. Sunt afectaţi predominant copiii, dar simptomatologia poate apărea şi la vârsta adultă. Boala este mai frecventă la pacienţii de sex masculin. Etiologia purpurei nu este deocamdată cunoscută, însă se pare ca există anumiţi factori de risc pentru apariţia acesteia: agenţi infecţioşi (Streptococ, parvovirusuri), înţepături de insecte, alimente sau medicamente (antibiotice, antihistaminice). Sus ceptibilitatea la PHS poate avea origine genetică. Numeroase rapoarte sugerează că deficienţa complementului (C4) datorată deleţiei genelor ce codează sinteza acestei fracţiuni determină predispoziţia la nefropatie IgA şi la nefrita Henöch-Schönlein.

Henöch-Schönlein purpura is a small-vessel vasculitis characterized by an abnormal response of the immune system: immunoglobulin A (IgA), C3 and immune complex deposition in arterioles, capillaries and venules. The syndrome is mostly seen in children, but it may affect people of any age. It is more common in boys than in girls. The etiology of Henöch-Schönlein purpura is unknown. Multiple infectious agents as well as drugs, foods, and insect bites may trigger Henöch-Schönlein purpura. Susceptibility to HSP may have a genetic origin. Several reports suggest that deficiency of complement 4 (C4) from deletion of C4 genes predisposes patients to IgA nephropathy and HSP nephritis.
