CONSTIPATIA CRONICA LA COPIL. ACTUALITATI IN EVALUARE SI MANAGEMENT

REZUMAT

Constipaţia este un fenomen comun în practica pediatrică. Prevalenţa sa este în creştere, fiind prezentă, conform diferitelor statistici, la 0,7-29,6% dintre copii. Constipaţia este întâlnită la toate grupele de vârstă, cu severitate şi durată variabile, de la forme uşoare şi de scurtă durată la forme severe şi de lungă durată, cu impactare fecală şi in continenţă fecală. Acest articol revede aspectele epidemiologice, clinice şi terapeutice ale constipaţiei la copil, funcţională în majoritatea cazurilor.

ABSTRACT

Constipation is a common problem in childhood, with a children prevalence varying between 0.7% and 29.6%. Constipation is encountered in all pediatric age groups, with variably severity and duration, from mild and short to severe and chronic forms, with faecal impact and encopresis. This article review the epidemiological, clinical and therapeutically aspects of children constipation, functional in the majority of cases.

CONSIMTAMANTUL INFORMAT PENTRU ENDOSCOPIA GASTROINTESTINALA IN PEDIATRIE

REZUMAT

Consimţământul informat a devenit o componentă majoră, dar şi obligatorie în practica medicală contemporană, numeroase foruri şi publicaţii subliniind comandamentele sale etice şi psihologice, ca şi modalităţile de obţinere şi mai ales implicaţiile de ordin legal. Implicaţiile acestei proceduri în cazul pediatriei sunt complexe, câteodată confuze sau insuficient intelese. Astfel dacă în cazul consultului clinic sau al actelor medicale simple, con sim- ţământul poate fi considerat implicit, în situaţia unor manevre provocatoare de disconfort psihic sau fizic, investigaţii invazive – în care este inclusă şi endoscopia sau acte terapeutice presupunând posibilitatea unor ris curi sau complicaţii, obţinerea unui consimtământ informat din partea parinţilor sau aparţinătorilor legali şi respectiv a unui acord (accept) din partea unui copil evoluat din punct de vedere intelectual, psihologic şi social devine mandatorie. Trebuie totuşi clarificate nuanţele nu numai semantice între consimţământ şi acord, ponderea fiecaruia din acestea în decizia finală, discernământul pacientului minor la diferite grupe de vârstă şi în egală măsură rolul şi locul medicului curant în astfel de circumstanţă.

INFORMED CONSENT IN PEDIATRIC GASTROINTESTINAL ENDOSCOPY

ABSTRACT

Informed consent has become a major, but also mandatory component in medical praxis nowadays. A great number of forums and publications emphasize its ethical and psychological commands, the most adequate way to obtain it and especially its legal implications. In pediatrics, the implications of this procedure are complex, sometimes confusing or not well understood. Thus, in clinical consultations or simple medical acts, the consent could be seen as implicit; when we talk about manoeuvres that lead to psychological or physical discomfort, invasive investigations, including endoscopy or therapeutical acts with possible risks or complications, informed consent from parents or legal tutors, respectively an intellectually, psychologically and socially evolved, educated child’s consent (acceptance) becomes mandatory. We must, however, clarify the nuances between consent and acceptance, not only from the semantic point of view, the proportion of either of the two terms in the final decision, the minor patient’s ability to discern at different age stages and finally the role and the importance of the doctor in such decision makings.

PRINCIPIILE MONITORIZARII COMBINATE IMPEDANT-PH-METRIE LA COPII: INDICATII, AVANTAJE SI LIMITARI

REZUMAT

Metoda de investigaţie standard în cazul suspiciunii unui reflux gastroesofagian este reprezentată de pH-metria esofagiană. Cu toate acestea, marea majoritate a episoadelor de reflux la sugari şi copii sunt slab-acide (pH>4), iar simptomele extradigestive pot fi declanşate de către episoade de reflux atât acid, cât şi slab-acid. Pentru a depăşi aceste aspecte, o nouă metodă a fost dezvoltată în ulltimii douăzeci de ani şi supusă evaluării ştiinţifice: impedanţa esofagiană. În combinaţie cu pH-metria esofagiană standard, aceasta aduce mult mai multe informaţii decât simpla pH-metrie, deoarece permite analiza refluxului non-acid şi a asocierii temporale dintre acesta şi simptome. Informaţia analizată precizează compoziţia bolusului (gaz, fluid, solid), înălţimea, du rata refluxului şi timpul de clearance al bolusului. Deşi nu sunt încă disponibile valori pentru limitele normalului în populaţia pediatrică, impedanţ-pH-metria promite să devină noul gold-standard în investigarea refluxului gastroesofagian.

PRINCIPLES OF COMBINED ESOPHAGEAL IMPEDANCE-PH MONITORING IN CHILDREN: INDICATIONS, BENEFITS AND LIMITATIONS

ABSTRACT

The standard test for suspected gastroesophageal reflux is pH monitoring. However, the majority of gastroesophageal reflux episodes in infants and children are nonacidic (pH > 4) and extraesophageal symptoms apparently can be caused by both acidic (pH < 4) and nonacidic reflux. In order to overcome these aspects, a new technique has been developed and placed under scrutiny in the past twenty years: multichannel intraluminal impedance. Combined with standard pH recording, it provides more information than simple pH measurement because it allows the study of non-acid reflux and the temporal association between symptoms and nonacid reflux. Data can be analyzed for bolus composition (gas, fluid, solid) and reflux height, duration, and clearance. Although normal values for the pediatric population are not yet available, combined multichannel intraluminal impedance and pH measurement has the potential to become the new “gold standard” for gastroesophageal reflux testing
