Eradication of Helicobacter Pylori infection in children: current matters and perspectives

ABSTRACT

The strategies used to treat Helicobacter pylori infection in children derives mostly from recommendations based on clear consents in adult patients. The triple therapy that associates proton pump inhibitors (PPI), clarithromycin and amoxicillin is used on a large extent all over the world; beside this, other associations came out based either on PPI or bismuth salts. Nowadays, new drugs were introduced in anti-bacterial schemes, alongside esomeprazole, pantoprazole or lansoprazole being azithromicine and fluoroquinolones (in older children). At the same time, second line therapies were suggested in order to treat relapse or bacterial reinfection and these are quadruple associations based on bismuth salts or regimens including fluoroquinolones; the introduction of sequential therapy in children also represents a promising alternative. Concerning prophylaxy, the oral or intramuscular vaccine (the latter being in an advance clinical trial), should be able to contribute in the future to decrease the incidence of H.pylori infection, gastritis, ulcer and gastric cancer.

ERADICAREA INFECTIEI CU HELICOBACTER PYLORI LA COPIL: ACTUALITATI SI PERSPECTIVE

REZUMAT

Strategiile utilizate în tratamentul infecţiei cu Helicobacter pylori la copii derivă în mare parte din schemele de tratament stabilite de consensuri clare la pacienţii adulţi. Tripla terapie ce asociază inhibitori de pompă protonică (IPP), claritromicina şi amoxicilina este utilizată pe scară largă în întreaga lume; alături de aceasta s-au impus şi alte asocieri bazate fie pe IPP, fie pe săruri de bismut. La ora actuală au fost introduse noi preparate în schemele antibacteriene, alături de esomeprazol, pantoprazol sau lansoprazol aflându-se azitromicina şi fluorochinolonele (la copiii mari). În acelasi timp au fost propuse terapii de linia a doua, rezervate cazurilor de recădere sau reinfecţie bacteriană, acestea fiind reprezentate de asocieri cvadruple bazate pe săruri de bismut sau de regimuri ce includ fluorochinolone; nu în ultimul rând, introducerea terapiei secvenţiale la copii reprezintă o alternativă promiţătoare. Pe plan profilactic, vaccinarea orală sau intramusculară, aceasta din urmă aflată în faze avansate de testare clinică, va putea contribui în viitor la scăderea incidenţei infecţiei cu H.pylori, a gastritei, ulcerului şi cancerului gastric.

Upper digestive tract manifestations inCRF patients on dialysis

REZUMAT

Background: CRF patients are prone to a wide range of complications: gastroesophageal reflux disease, esophagitis, gastropathy, gastritis and duodenitis, gastric and duodenal ulcer, angiodysplasia.

Objectives: the study’s aim was to highlight the gastro-duodenal symptoms, the frequency of pathological modifi cations of eso-gastro-duodenal mucous membrane and the presence of H.pylori in CRF children receiving dialysis.

Material and methods: the study included 55 children admitted to the IVth Pediatric Nephrology Clinic of the Emergency Clinical Hospital “Sf Maria” Iasi, all receiving dialysis, to which was applied a systematized protocol: objective and subjective symptoms and the results of upper gastrointestinal endoscopy. The study included patients with mucosal changes detected endoscopically.

Conclusions: Pathological modifications of the upper digestive tract mucous membrane in children on chronic dialysis are inconstant, suggesting the need for endoscopy in each case. H.pylori was found in 24/55 (43.63%) biopsies and points out the relatively frequent association between gastro-duodenal lesions and H.pylori in chronic dialysis patient.

MANIFESTARILE TUBULUI DIGESTIV SUPERIOR LA PACIENTUL CU IRC DIALIZAT

REZUMAT

Introducere: bolnavii cu IRC sunt susceptibili la o gamă largă de complicaţii: reflux gastro-esofagian, esofagită, gastropatie, gastrită şi duodenită, ulcer gastric şi duodenal, angiodisplazie. Obiective: studiul a constat în evidenţierea simptomelor gastro-duodenale, a frecvenţei modificărilor patologice ale mucoasei eso-gastro-duodenale şi a prezenţei H.pylori la copiii cu IRC aflaţi în program de dializă. Material şi metodă: lotul de studiu a fost format din 55 de copii internaţi în Clinica a IV-a Nefrologie, aflaţi în program de substituţie renală cronică, la care s-a aplicat un protocol ce a sistematizat: simptomele obiective şi subiective şi rezultatele endoscopiei digestive superioare. Lotul cuprinde bolnavii cu modificări ale mucoasei depistate endoscopic. Concluzii: modificările patologice ale mucoasei eso-gastro-duodenale la copilul dializat cronic se reflectă clinic inconstant, ceea ce sugerează necesitatea endoscopiei pentru fiecare caz. Prezenţa H.pylori la 24/55 (43,63%) cazuri biopsiate evidenţiază asocierea relativ frecventă între leziunile gastro-duodenale şi H.pylori la pacienţii dializaţi cronic.

The treatment of gastroduodenal ulcer in children

ABSTRACT

The gastroesophageal ulcer is not a rare pathology in paediatrics as it was thought. The paper approaches the essential aspects of the dietetic and drug therapy. The medical treatment uses a large pharmacologic arsenal: antiacids, gastric protectors, H2 receptors antagonists, proton pump inhibitors, plus, for the eradication of Helicobacter pylori, antibiotherapy with 2 antibiotics.

REZUMAT

Ulcerul gastroduodenal este o patologie nu atât de rară în patologia pediatrică pe cât se credea. Lucrarea abordează aspectele esenţiale ale terapiei dietetice şi medicamentoase. Tratamentul medicamentos foloseşte un întreg arsenal farmacologic: antiacide, protectoare gastrice antagonişti de H2 receptori, inhibitori de pompă protonică, iar în plus, pentru eradicarea infecţiei cu Helicobacter pylori, antibioterapia cu 2 antibiotice.

Pompe disease: criteria for diagnosis and treatment

ABSTRACT

Pompe disease is a rare neuromuscular disorder, with autosomial recessive transmition due to deficiency of lysosomal enzymatic acid alpha glucosidase (GAA) with accumulation of glycogen in tissues. The infantileonset form is the most severe with hypotonia and cardiomyopathy with death in the first years of life. The diagnosis may be confirmed by analysis of GAA enzyme activity at fibroblasts level by muscle or skin biopsy. This method is realized by invasive procedures and results are obtained after few weeks thus delaying diagnosis. In the last few years assays on blood cells and dried blood spots have been developed, method that are not invasive and can help at early diagnostic of Pompe disease. Treatment is represented by enzyme replacement therapy with alglucosidase alfa. This treatment changes the natural course of the disease. In patients with infantile onset enzyme replacement therapy prolongs survival and in those with delay onset improve walking distance and stabilize pulmonary function.

BOALA POMPE: CRITERII DE DIAGNOSTIC SI TRATAMENT

REZUMAT

Boala Pompe este o afecţiune neuromusculară rară, cu transmitere autosomal recesivă, datorată unui deficit enzimatic lizozomal de acid alfa glucozidază (GAA) care determină acumularea de glicogen în ţesuturi. Forma cu debut precoce, în perioada de sugar, este cea mai severă şi se manifestă prin hipotonie şi cardiomiopatie cu deces în primii ani de viaţă. Diagnosticul poate fi confirmat printr-o evaluare cantitativă a activităţii enzimei GAA la nivelul fibroblaştilor, prin biopsie musculară sau cutanată. Acestă metodă se realizează prin proceduri invazive, iar rezultatele se obţin după câteva săptămâni, ceea ce întârzie diagnosticul. În ultimii ani s-a dezvoltat o nouă tehnică de diagnostic care cercetează activitatea enzimatică pe mostre de sânge uscat, care nu este invazivă şi care permite diagnosticul precoce al bolii Pompe. Tratamentul constă în terapie de substituţie enzimatică cu alglucozidază alfa, care modifică evoluţia naturală a bolii, la pacienţii cu debut infantil prelungind supravieţuirea, iar la cei cu debut tardiv îmbunătăţind distanţa parcursă în mers şi stabilizând funcţia pulmonară.

Schönlein-Henoch purpura with neurological manifestations

ABSTRACT

Neurological symptoms represent possible manifestations of Schönlein-Henoch purpura. The authors present two children diagnosed with Schönlein-Henoch purpura. In booth cases the onset of disease was with neurological symptoms (epileptic seizures). In booth cases was observed behavioral problems and interictal EEG revealed abnormalities. In one case the neuroimaging exam revealed cerebral ischemic lesions. The authors consider that Schoenlein-Henoch purpura can involve the nervous system and review of the literature indicates that epileptic seizures and behavioral changes are the most frequent neurologic manifestations of this disease, followed by headache, focal neurologic deficits, mononeuropathies and polyradiculoneuropathies

PURPURA SCHONLEIN-HENOCH CU MANIFESTARI NEUROLOGICE

REZUMAT

Simptomele neurologice reprezintă manifestări posibile ale purpurei Schönlein-Henoch. Autorii prezintă doi copii diagnosticaţi cu purpură Schönlein-Henoch la care semnele de debut au fost neurologice, fiind reprezentate de crize epileptice. În ambele cazuri s-au asociat tulburări de comportament, iar EEG intercritic a evidenţiat grafoelemente patologice. Într-un caz examenul neuroimagistic cerebral a obiectivat leziuni ischemice cerebrale. Autorii confirmă că purpura Schönlein-Henoch poate interesa sistemul nervos, iar revederea literaturii arată că cele mai frecvente manifestări neurologice ale acestei boli sunt crizele epileptice şi tulburările de comportament, urmate de cefalee, deficite neurologice focale, mononeuropatii şi poliradiculoneuropatii
