The Evaluation of Prenatal Hydronephrosis and Postnatal Management

ABSTRACT

The prenatal ultrasound advent has markedly changed the evaluation and management of hydronephrosis. If the classical solution to hydronephrosis was the surgical correction based on the premise that pelvic dilatation means pelvic obstruction, nowadays a more conservative approch is reccomended. Even so, the great dilemma remains to differentiate the clinically significant hydronephrosis from the transient one and to choose the optimal way to manage this cases.

REZUMAT

Evoluţia diagnosticului prenatal a adus modificări importante în evaluarea şi managementul hidronefrozei. Dacă soluţia clasică implica intervenţie chirurgicală corectoare plecând de la premiza că dilatarea pielică înseamnă obstrucţie, în prezent abordarea conservatoare este regula. Marea provocare este reprezentată în continuare de diferenţierea hidronefrozei clinic semnificative de cea tranzitorie şi alegerea căii optime de management al acestor cazuri.

Portal hypertension in children

ABSTRACT

The diagnosis and medical and surgical therapy of portal hypertension in children have suffered numerous changes over the last decades, being accompanied by an improvement in the prognosis of these patients. The etiology of portal hypertension occurrence has also undergone many changes consisting of an increase in the number of cases of hypertension of hepatic involvement, to the detriment of those with pre-hepatic involvement. This may be account for by higher rates of survival in patients suffering from conditions responsible for the occurrence of hepatic portal vein occlusion (biliary and hepatic atresia in the neonates, etc). An accurate and early diagnosis of these conditions, as well as the proper etiology and portal vein blocking site identification, will allow an accurate medical and/or surgical approach, an increase in the life quality and survival rates of these patients.

HIPERTENSIUNEA PORTALA LA COPIL

REZUMAT

Diagnosticul şi tratamentul medical şi chirurgical al hipertensiunii portale la copil a cunoscut multiple modificări în ultimele decade, cu o îmbunătăţire în ceea ce priveşte prognosticul acestor pacienţi. De asemenea, etiologia apariţiei hipertensiunii portale a suferit numeroase schimbări prin creşterea proporţiei cazurilor de hipertensiune de cauză hepatică, în detrimentul celor de cauză prehepatică, datorită creşterii supravieţuirii în cazul unor afecţiuni ce sunt responsabile de apariţia obstrucţiei portale hepatice (atrezia de căi biliare, hepatite neonatale etc). Diagnosticul corect şi precoce al acestor situaţii, ca şi identificarea corespunzătoare a etiologiei şi localizării barajului portal vor duce la o abordare medicală şi/sau chirurgicală corectă, cu creşterea calităţii vieţii şi supravieţuirii acestor pacienţi.

Colecistita acută perforată alitiazică, frecventă la adult, este o entitate rar întâlnită la copil. Simptomatologia clinică necaracteristică, aspectele imagistice frecvent neconcludente, fac ca diagnosticul de colecistită acută alitiazică să fie deseori stabilit intraoperator. Această condiţie trebuie totuşi să fie evocată în cadrul diagnosticului diferenţial al unui sindrom peritonitic la copil.

ACUTE ACALCULOUS CHOLECYSTITIS WITH GALLBLADDER PERFORATION IN CHILDREN – CASE REPORT

ABSTRACT

Although relatively common in adult pathology, acute acalculous cholecystitis with gallbladder perforation is a rather infrequent entity in pediatric practice. In many cases, its unspecific clinical symptoms and the often inconclusive medical imagining results lead to the diagnosis of acute acalculous cholecystitis being set intraoperatively. Nevertheless, this condition should be considered when setting the differential diagnosis of a peritonitis syndrome in children.

AFECTAREA HEPATICA LA COPIII CU FIBROZA CHISTICA

Laura Mihaela Trandafir, Iulia Straticiuc Ciongradi, Ginel Baciu and Dana Teodora Anton Păduraru

REZUMAT

Afectarea hepatică reprezintă o manifestare precoce la copiii cu fibroză chistică (FC). Spectrul manifestărilor clinice în afectarea hepato-biliară din FC cuprinde colestaza neonatală, steatoza hepatică, fibroza hepatică, litiaza biliară, ciroza biliară focală şi ciroza multilobulară, cu sau fără hipertensiune portală. Modificarea testelor de evaluare a funcţiei hepatice este inconstantă şi nu se corelează cu severitatea leziunilor hepatice. Diagnosticarea bolii hepatice în FC necesită prezenţa a cel puţin două din cele patru criterii de diagnostic: manifestări clinice, modificarea testelor funcţionale hepatice, ecografice şi anatomopatologice. Screeningul anual pentru suferinţa hepatică este recomandat pentru diagnosticarea bolii asimptomatice şi iniţierea precoce a tratamentului cu acid ursodeoxicholic. Ameliorarea funcţiei hepatice influenţează calitatea vieţii şi creşte rata supravieţuirii la pacienţii cu FC.

LIVER DISEASE IN CHILDREN WITH CYSTIC FIBROSIS

ABSTRACT

Liver disease is an early complication in children with cystic fibrosis (CF).The clinical manifestations in hepatobiliary disease of CF include neonatal cholestasis, liver steatosis, liver fibrosis, biliary lithiasis, focal biliary cirrhosis and multilobular cirrhosis, with or without portal hypertension. Changes in the tests evaluating the liver function are inconsistent and are not correlated with the severity of the liver lesions. The diagnosis of liver disease in CF requires the presence of at least two of the following four diagnosis criteria:clinical manifestations, pathologic liver function tests, ultrasound and histologic changes.The annual follow-up to evaluate the liver function is s recommended for diagnosis of asymptomatic liver disease and early initiation of treatment with ursodeoxycholic acid. The improvement of the liver function influences life quality and increases the survival rate in patients with CF
