PROVOCARI DE DIAGNOSTIC IN PNEUMONIA PEDIATRICA

REZUMAT

Pneumonia este o formă de infecţie respiratorie acută care afectează plămânii şi care este cauza principală de mortalitate în rândul copiilor la nivel mondial. Pneumonia poate apărea la orice vârstă, dar este mai frecventă la copiii mici. Etiologia ei este adesea dificil de determinat, criteriile de diagnostic fiind adesea prezumptive şi bazându-se pe date epidemiologice, clinice, de laborator şi radiologice. Autorii prezintă trei cazuri de pneumonie diagnosticate la vârste diferite, subliniind particularităţile clinice, imagistice şi evolutive.

CHALLENGES IN THE DIAGNOSIS OF PEDIATRIC PNEUMONIA

ABSTRACT

Pneumonia is a form of acute respiratory infection that affects the lungs and it is the leading cause of illness and death in children worldwide. Pneumonia can occur at any age, but it is more common in younger children. The etiology of pneumonia is often difficult to identify and the diagnostic criteria is based on corroborating epidemiological, clinical, laboratory and radiologic investigations. The authors present three cases diagnosed with pneumonia at different ages, underlying the clinical, imagistic and evolutive particularities.

Evolutive and clinical issues in acute infant pneumonia

ABSTRACT

Infectious pneumonia are the most common products of viruses and bacteria. In children etiology is dominated by viral infections that are up to 90% of all child pneumonia The following paper is the result of a retrospective study performed on a lot of 165 infants with acute pneumonia that were admitted to the Pediatric Clinic of Filantropia Hospital Craiova between the 1st of January – the 1st of July 2010, our purpose being to follow the frequency and the current evolution of acute infant pneumonia and also to assess the risk factor that leads to the installation of the respiratory disorders. In the cases we studied, some with severe forms and others with prolonged evolution, acute pneumonias healed without sequelae. The forms that necessitated more than 10 day hospitalization required additional investigations in order to exclude complications, affections or accidents. After having excluded other factors, we correlated the prolonged evolution of these cases with particularities regarding the age and the immunodefi ciency (dystrophy, rickets).

ASPECTE CLINICO-EVOLUTIVE IN PNEUMONIA ACUTA LA SUGAR

REZUMAT

Pneumoniile infecţioase sunt produse cel mai frecvent de virusuri şi bacterii, la copil etiologia fiind dominată de infecţiile virale, care reprezintă până la 90% din toate pneumoniile copilului. Prezenta lucrare este rezultatul unui studiu retrospectiv efectuat pe un lot de 165 de sugari cu pneumonie acută care au fost internaţi în Clinica de Pediatrie a Spitalului Filantropia din Craiova în perioada 1 ianuarie-1 iulie 2010, în care ne-am propus să urmărim frecvenţa şi tendinţa actuală de evoluţie a pneumoniei acute la sugar, precum şi factorii de risc ce duc la instalarea tulburărilor respiratorii. Pneumoniile acute la cazurile studiate s-au vindecat fără sechele, atât formele grave, cât şi cele cu evoluţie prelungită. Formele cu spitalizare de peste 10 zile au necesitat investigaţii suplimentare pentru excuderea unor complicaţii, afecţiuni sau accidente. La aceste cazuri evoluţia prelungită a fost corelată cu particularităţile legate de vârstă şi terenul imunodeficitar (distrofici, rahitici) după ce s-au exclus alte cauze.

Tracheobronchial foreign bodies in children – diagnostic and therapeutic aspects

ABSTRACT

Objective. The paper aimed to study of diagnostics and therapeutics of tracheobronchial foreign body suspicions in children. Materials and method. The retrospective study was performed on 106 cases with suspicion of tracheobronchial foreign body (TBFB) admitted and treated in the E.N.T Clinic of Craiova County Hospital between 2006 and 2010. Results. Out of 77 cases (72%) confirmed trough bronchoscopy, 68% were males, most frequently aged between 1-3 years (63.63%). The penetration syndrome was present in 88.31%. The most frequent symptom was coughing, in 96.10 percent (74 cases), followed by dyspnea with 81.81 % and cyanosis in 58.44 %. Within the study lot, the most frequently encountered radiological sign was: emphysema (27%), followed by atelectasis – (18%). The organic foreign bodies prevailed within the study lot (83.11%) followed by an-organic foreign bodies (16.89%). Conclusions. The evolution and prognosis of inhaling a tracheobronchial foreign body, depends on the precocity of diagnosis and therapeutic intervention.

CORPII STRAINI TRAHEOBRONSICI LA COPIL – ASPECTE DIAGNOSTICE SI TERAPEUTICE

REZUMAT

Obiectiv. Lucrarea a avut ca scop studiul aspectelor diagnostice şi terapeutice în suspiciunile de corp străin traheobronşic la copil.

Material şi metodă. Studiul retrospectiv a fost efectuat pe 106 cazuri cu suspiciune de corp străin traheobronşic (CSTB), internate şi tratate în Clinica ORL a Spitalului Clinic Judeţean de Urgenţă din Craiova în intervalul dintre anii 2006 şi 2010.

Rezultate. Din 77 cazuri (72%) confirmate în urma bronhoscopiei, 68% au fost înregistrate la băieţi. La grupa de vârstă 1-3 ani au fost întâlnite majoritatea cazurilor (63,63%). Sindromul de penetraţie a fost întâlnit în 88,31%. Cel mai frecvent simptom a fost tusea, într-un procent de 96,10, urmat de dispnee cu 81,81% şi cianoza în 58,44%. Cel mai frecvent semn radiologic întâlnit a fost emfizemul (27%) urmat de atelectazie (18%). Corpii străini organici predomină în lotul studiat (83,11%) urmaţi de cei anorganici (16,89%).

Concluzii. Evoluţia şi prognosticul aspirării de corp străin traheo-bronşic depind de precocitatea diagnosticului şi a intervenţiei terapeutice.
