OBEZITATEA LA PRESCOLAR – STUDIU CAZUISTIC

REZUMAT

Obezitatea primară la copii, una dintre cele mai frecvente boli de nutriţie din lume, prezintă particularităţi dependente de vârstă privind factorii de risc, măsurile de prevenţie şi tratament. Obiectivul prezentului studiu a fost reprezentat de evaluarea pacienţilor de vârstă preşcolară diagnosticaţi cu obezitate primară în Clinica III Pediatrie a Spitalului de Urgenţă pentru Copii „Sf. Maria“ Iaşi în vederea identificării debutului supraponderalităţii/obezităţii şi a factorilor de risc prenatali, postnatali precoce, acumulaţi în primii 6 ani de viaţă implicaţi în producerea obezităţii. Concluzionăm că accentul în combaterea obezităţii copilului trebuie pus pe profilaxia acesteia, deosebit de importante fiind măsurile pentru promovarea unei alimentaţii sănătoase şi pentru combaterea sedentarismului.

OBESITY IN PRESCHOOL CHILDREN

ABSTRACT

Primary obesity in children, one of the most common nutrition diseases in the world, shows particularities depending on the age regarding the risk factors, prevention and treatment. The aim of the study was the assessment of preschool patients diagnosed with primary obesity in the 3rd Clinic of Pediatrics, “Sf. Maria” Children’s Emergency Hospital Iasi, in order to identify the onset of overweight/obesity and the prenatal and postnatal risk factors from the first 6 years of life involved in causing obesity. We conclude that in order to combat obesity the emphasis should be on prevention, particularly important being the measures that promote healthy eating and combat sedentariness.

POLIMORFISMUL MANIFESTARILOR CLINICE IN BOALA CELIACA LA COPIL

REZUMAT

Boala celiacă (BC) reprezintă o afecţiune sistemică autoimună produsă prin sensibilizarea la gluten la pacienţii cu predispoziţie genetică. Manifestările clinice ale BC sunt extrem de variate, de la sindromul tipic de malabsorbţie intestinală (caracterizat prin diaree cronică, meteorism abdominal şi malnutriţie) la simptome atipice care pot afecta orice sistem sau organ: constipaţie cronică, hepatocitoliză persistentă, anemie refractară la tratamentul cu fier, dureri abdominale recurente, afecţiuni neurologice, defect al smalţului dentar. Autorii prezintă patru pacienţi de vârstă pediatrică diagnosticaţi cu diferite forme atipice de boală celiacă: forma cu dureri abdominale recurente, cu sindrom de hepatocitoliză şi formă cu constipaţie cronică, toate însoţite de falimentul creşterii şi un caz de diabet zaharat tip I asociat cu boală celiacă. În concluzie, este necesară cunoaşterea tuturor formelor clinice ale bolii celiace de către medicul pediatru, gastroenterolog şi medicul de familie pentru a diagnostica boala la vârsta copilăriei şi, astfel, să prevină complicaţiile pe termen lung, respectiv osteoporoza, infertilitatea si limfomul intestinal.

POLYMORPHISM OF THE CLINICAL SIGNS OF CELIAC DISEASE IN CHILDREN

ABSTRACT

Celiac disease (CD) is an autoimmune systemic condition caused by sensitivity to gluten in genetically predisposed patients. The clinical signs of CD are extremely diverse, ranging from the typical intestinal malabsorption syndrome (characterized by chronic diarrhea, abdominal distension and malnutrition) to atypical symptoms that may involve any system or organ: chronic constipation, increased level of liver enzymes, iron deficiency anemia, recurrent abdominal pain, neurological conditions, tooth enamel erosion. The authors report four pediatric cases diagnosed with various atypical forms of celiac disease: the form associated with recurrent abdominal pain and increased level of liver enzymes, and the form associated with chronic constipation, all accompanied by failure to thrive and one case of type I diabetes mellitus associated with celiac disease. To conclude with, pediatricians, gastroenterologists and general practitioners should be familiar with all the clinical forms of coeliac disease in order to be able to diagnose this childhood disease and thus prevent long-term complications like osteoporosis, infertility and intestinal lymphoma.

REZUMAT

Diabetul asociat fibrozei chistice este o entitate distinctă comparativ cu diabetul zaharat tip 1 şi 2, dar care are şi simptome caracteristice acestora. Odata cu prelungirea speranţei de viaţa, şi prevalenţa diabetului a crescut, asociindu-se cu un declin mai sever al funcţiei pulmonare şi cu stare nutriţională mai precară comparativ cu ale pacienţilor cu fibroză chistică, dar fără diabet. Autorii prezintă date actuale cu privire la prevalenţa şi fiziopatologia bolii, tabloul clinic, examenele utile în stabilirea diagnosticului, posibilităţile terapeutice şi prognosticul bolii. Concluzionăm că diagnosticarea precoce şi intervenţiile terapeutice adecvate pot diminua impactul negativ al diabetului asupra funcţiei pulmonare şi satusului nutriţional în FC.

CYSTIC FIBROSIS-RELATED DIABETES

Dana-Teodora Anton-Paduraru, Carmen Oltean, Maria-Liliana Iliescu, Ginel Baciu and Laura Mihaela Trandafir

ABSTRACT

Cystic fibrosis-related diabetes is an entity distinct from diabetes mellitus type 1 and 2, but with symptoms characteristic to them. Along with the extension of the life expectancy, the prevalence of diabetes increased in association with a more severe decline of the lung function and a poorer nutritional status in comparison to that of people with cystic fibrosis but without diabetes. The authors present current data regarding the prevalence and physiopathology of the disease, the clinical picture, and the useful examinations in establishing the diagnosis, the therapeutic possibilities and disease prognostic. We conclude that the early diagnosis and appropriate therapeutic interventions may diminish the negative impact of diabetes on the lung function and the nutritional status in cystic fibrosis.

REZUMAT

Vegetarianismul poate avea implicaţii asupra creşterii copiilor şi adolescenţilor. Există diferite tipuri de diete vegetariene: unele pot fi adecvate pentru copii, altele deficitare în nutrienţi esenţiali. Dacă o dietă vegetariană este realizată adecvat şi include alimente fortifiate, poate promova starea de sănătate şi reduce riscul de boli cronice majore. Restricţia sau excluderea produselor de natură animală poate conduce la aport scăzut de nutrienţi esenţiali.

ABSTRACT

Vegetarianism may have implications on the growth of children and adolescents. There are different types of vegetarian diets: some may be suitable for children, other deficient in essential nutrients. If a vegetarian diet is done properly and includes fortified foods it can promote health and reduce risk for major chronic diseases. Restriction or exclusion of products of animal nature can lead to low intake of essential nutrients
