Cognitive deficits in children – candidate endophenotypes for schizophrenia?

ABSTRACT

There is an intrinsic relationship between cognitive impairments and vulnerability to schizophrenia. Objective: to evaluate if the cognitive deficits are present to the unaffected childrens of schizophrenic patients and this cognitive deficit could be a good predictor for later psychosis. Methods: the study was of observational prospective type, lasting for 3 years was made on 2 groups – 50 childrens (35 boys, 15 girls; age range = 6-16 years) who had at least one parent meeting Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition (DSM-IV TR) criteria for schizophrenia and 10 normal controls, who had healthy parents. Results: There was an important cognitive deficit to the group of childrens with an increased risk for the development of schizophrenia, especially in executive functions. Childhood neurocognitive deficits were related to social skill deficits in adolescence and social isolation, suggesting a causal relationship with at least the interpersonal component of the vulnerability. Conclusions: Cognitive impairment of childrens with increase risk for schizophrenia may represent a valid endophenotyp, which in turn may be a very effective for the study of a disease of a complexity as well as schizophrenia. Also, early cognitive intervention might reduce multiple vulnerability components.

REZUMAT

Există o relaţie intrinsecă între deficitul cognitiv din schizofrenie şi vulnerabilitatea pentru această tulburare. Obiectiv: de a evalua dacă deficitul cognitiv este prezent la copiii pacienţilor diagnosticaţi cu schizofrenie şi dacă acest deficit cognitiv manifest ar putea reprezenta un predictor pentru dezvoltarea unei psihoze viitoare. Metode: studiul a fost unul de tip prospectiv observaţional, cu o durată de 3 ani, asupra a două loturi de studiu – 50 de copii (35 de băieţi şi 15 fete, cu media de vârstă între 6 şi 16 ani), care aveau cel puţin un părinte care îndeplinea criteriile de diagnostic pentru schizofrenie, conform DSM-IV TR, şi 10 indivizi de control, care aveau ambii părinţi sănătoşi. Rezultate: s-a observat că există un deficit cognitiv semnificativ din punct de vedere statistic la grupul de copii cu risc crescut de dezvoltare a schizofreniei, în special în ceea ce priveşte funcţiile executive, comparativ cu grupul de control. Deficitele cognitive din copilărie au fost corelate cu performanţe scăzute la nivel social în adolescenţă, izolare socială la vârsta adultă, sugerând astfel o relaţie cauzală cel puţin cu componenta interpersonală a vulnerabilităţii. Concluzii: deficitul cognitiv la copiii cu risc crescut de dezvoltare a schizofreniei ar putea reprezenta un endofenotip, care ar putea avea un rol important în studiul unei afecţiuni de o asemenea complexitate cum este schizofrenia. De asemenea, intervenţia cognitivă precoce ar putea influenţa multiplele componente ale vulnerabilităţii pentru schizofrenie.
