Spinning top urethra and voiding dysfunction in children

ABSTRACT

Voiding dysfunction is a common clinic complaint during childhood, frequently associated with urinary tract infection, vesicoureteric reflux and chronic constipation.

The aim of the study is to evaluate the existence of an association between the symptoms of voiding dysfunction and the cystoureterogram aspect of spinning top urethra (STU).

Material and method. The study was performed during January 2005 and December 2009 in the IIIrd Pediatric Clinic, Cluj-Napoca. The study included 105 patients with symptomatic voiding dysfunction. The patients were divided in two groups, according to the presents or absence of STU. They were investigated clinically as well as through imaging techniques by means of renourinary ultrasound and voiding cystourethrography.

Results. STU was present in 57.1% of the patients, more frequent in females. Urgency (82.5%) and urinary incontinence (75.3%) were statistically significant associated with STU. In the patients with voiding dysfunction and STU, recurrent urinary tract infection is independently associated with STU.

In conclusion, the cystourethrography aspect of STU is more frequent in females and statistically significant associated with voiding dysfunction symptoms and with recurrent urinary tract infection.

REZUMAT

Disfuncţia micţională este o manifestare clinică comună vârstei pediatrice. Este frecvent asociată cu infecţia urinară recurentă, constipaţia cronică şi refluxul vezicoureteral.

Scopul lucrării este de a evalua existenţa unei asocieri între simptomele disfuncţiei micţionale şi aspectul cistografic de spinning top urethra (STU).

Material şi metodă. Studiul s-a efectuat în Clinica Pediatrie III, Cluj-Napoca, în perioada ianuarie 2005- decembrie 2009. S-au luat în studiu un număr de 105 pacienţi cu simptomatologie de disfuncţie micţională, care au fost împărţiţi în două loturi, în funcţie de prezenţa sau nu a STU. Pacienţii au fost investigaţi clinic şi imagistic prin ultrasonografie renourinară şi uretrocistografie micţională retrogradă.

Rezultate. STU a fost prezentă într-un procent de 57,1%, fiind mai frecventă la sexul feminin. Urgenţa la micţiune (82,5%) şi incontinenţa urinară (75,3%) sunt factori independenţi asociaţi statistic semnificativ cu aspectul de STU. La pacienţii cu disfuncţie micţională şi STU, infecţia urinară recurentă este un factor independent asociat cu spinning top urethra.

În concluzie, la pacienţii cu disfuncţie micţională, aspectul cistografic de STU, este mai frecvent întâlnit la sexul feminin şi se asociază statistic semnificativ cu simptomele disfuncţiei micţionale şi infecţia urinară recurentă.

Kikuchi-Fujimoto disease – diagnosis problems

ABSTRACT

Kikuchi disease is a rare, benign, self-limiting disease of unknown etiology, which should be included in the diagnostic algorithm of cervical adenopathy. The certainty diagnosis is histopathological, specific changes including paracortical lymph node necrosis, histiocytes with crescent shaped nuclei and many apoptotic bodies. The case of a 13-year-old patient diagnosed with Kikuchi disease, a disorder rarely described at pediatric age, is presented.

REZUMAT

Boala Kikuchi este o afecţiune rară, benignă şi autolimitată, de etiologie necunoscută, care ar trebui inclusă în algoritmul diagnostic al adenopatiei cervicale. Diagnosticul de certitudine este histopatologic, modificările specifice fiind necroza paracorticală ganglionară, histiocitele cu nuclei în formă de semilună şi numeroşii corpi apoptotici. Se prezintă cazul unui pacient de 13 ani cu diagnosticul de boală Kikuchi, afecţiune rar descrisă la vârsta pediatrică.
