ASPECTE CLINICE SI TERAPEUTICE IN COARCTATIA DE AORTA SEVERA CU CIRCULATIE SISTEMICA DUCTAL DEPENDENTA LA NOU-NASCUT

REZUMAT

Coarctaţia de aortă are incidenţă de 0,6-0,8/1.000 nou-născuţi, manifestându-se clinic din perioada neonatală. La nou-născut, malformaţiile congenitale cardiace asociate cu insuficienţă cardiacă şi răsunet clinic încă din primele ore de viaţă au un prognostic rezervat, necesitând un diagnostic precoce şi o conduită terapeutică adecvată. Prezentăm un caz clinic de coarctaţie de aortă severă cu circulaţie sistemică ductal dependentă, diagnosticat din perioada neonatală. Tratamentul precoce cu prostaglandină E1 a permis menţinerea echilibrului hemodinamic, copilul beneficiind de corecţie chirurgicală în prima lună de viaţă.

CLINICAL AND THERAPEUTIC ASPECTS IN THE SEVERE AORTIC COARCTATION WITH DUCT DEPENDENT SYSTEMIC CIRCULATION IN THE NEWBORN

ABSTRACT

The aortic coarctation has an incidence of 0.6-0.8/1,000 newborn, with clinical manifestation starting with the neonatal period. In the newborn, the heart congenital malformations associated to heart failure and clinical response early in the first hours of life have a guarded prognosis, needing an early diagnostic and an adequate therapeutic conduct. We present a clinical case of severe aortic coarctation with duct dependent systemic circulation, diagnosed early in the neonatal period. The early treatment with Prostaglandin E1 allowed the maintenance of the haemodynamic balance, the newborn benefiting from surgical correction in the first month of life.

Malformaţiile congenitale de cord cu circulaţie pulmonară ducto-dependentă sunt cardiopatii cu manifestări severe neonatale şi degradare hemodinamică rapid progresivă. Simptomatologia evidentă de la naştere necesită un diagnostic corespunzător şi management terapeutic precoce prin menţinerea permeabilităţii canalului arterial.

ABSTRACT

The cardiac congenital malformations with duct dependent pulmonary circulation are cardiopathies with severe neonatal manifestations and quickly progressive hemodynamic degradation. The obvious semeiology at the birth requires an appropriate diagnostic and an early therapeutic management through the maintenance of the arterial channel permeability.

PROVOCARI DE DIAGNOSTIC IN TAHICARDIA VENTRICULARA LA COPIL

REZUMAT

Aritmiile cardiace primare sunt mult mai puţin frecvente la sugari şi copii decât la adulţi, simptomele prezentate putând fi vagi şi nespecifice. Deşi urgenţele reale din cauza aritmiilor instabile la copii sunt rare, este important să se identifice şi să se gestioneze în mod corespunzător cazurile. Prezentăm cazul unui copil, care se internează pentru simptomatologie respiratorie, la examenul clinic decelându-se o frecvenţă cardiacă de 214bpm. Copilul a devenit progresiv anxios, prezentând efort respirator. Studiul electrocardiogramei a stabilit diagnosticul de tahicardie ventriculară fasculară anterioară cu tendinţa la permanentizare. Evoluţia a fost favorabilă după efectuarea cardioversiei electrice şi tratament preventiv cu Verapamil.

DIAGNOSTIC CHALLENGES IN CHILD VENTRICULAR TACHYCARDIA

ABSTRACT

Primary cardiac arrhythmias are much less common in infants and children than in adults, their symptoms can be vague and nonspecific. Although real emergencies because of unstable arrhythmias in children are rare, it is important to identify and properly manage these cases. The study presents the case of a child who was admitted for respiratory symptoms, clinical examination discovering the heart rate of 214bpm. The child became progressively anxious, showing a respiratory effort. The study of electrocardiogram established the diagnosis of anterior fascicular ventricular tachycardia with a tendency to become permanent. The evolution was favorable after the electrical cardioversion and preventive treatment with Verapamil had been performed.

REZUMAT

Malformaţiile congenitale de cord cu circulaţie ducto-dependentă au o fi ziologie cardiovasculară complexă. După naştere, situaţia acestor copii este critică, iniţial sunt asimptomatici apoi rezistenţa vasculară pulmonară scade, ducând la degradarea hemodinamică progresivă. Identificarea cât mai precoce a acestor malformaţii şi iniţierea terapiei corespunzătoare permite menţinerea echilibrului cardio-vascular.

CLINIC AND THERAPEUTIC ASPECTS IN DUCTUS-DEPENDENT CONGENTITAL HEART DEFECTS – PART I

ABSTRACT

Congenital heart defects with ductus-dependent circulation have a complex cardiovascular physiology. After birth, the condition of these children is critical, they are asymptomatic and their pulmonary vascular resistance decreases leading to progressive hemodynamic deterioration. Early Identification of these defects and the start of appropriate therapy allow to maintain the cardiovascular balance.
